超声消融联合扩张成形和支架植入治疗下肢动脉完全闭塞症的临床疗效观察

摘要:
[摘要] 目的 探讨超声消融联合扩张成形和支架植入治疗下肢动脉完全闭塞症的临床疗效和安全性。方法 选取该院2012-02~2015-02收治的下肢动脉完全闭塞症患者67例,根据患者意愿分为观察组42例和对照组25例。对照组予以常规药物保守治疗,观察组予以超声消融联合扩张成形和支架植入治疗,观察对比两组临床疗效和终点事件发生情况。结果 观察组管腔通畅率为85.7%,明显高于对照组的44.0%(P<0.01);观察组病死率、截肢率、心血管事件发生率分别为2.4%、2.4%、4.8%,分别低于对照组的24.0%、24.0%、36.0%,组间比较差异均有统计学意义(P均<0.05)。观察组治疗后的踝肱指数(0.82±0.11)明显高于对照组(0.61±0.12),差异有统计学意义(P<0.01)。结论 超声消融联合扩张成形和支架植入治疗下肢动脉完全闭塞症的近期临床疗效确切,可有效疏通完全闭塞的动脉,有效改善下肢血供,降低终点事件发生率,但需远期进一步观察临床疗效。

Abstract:
[Abstract] Objective To explore the clinical efficacy and safety of ultrasound ablation combined with dilation and stent implantation in the treatment of lower extremity arterial occlusion.Methods Sixty-seven patients with total occlusion of the lower extremity from Februory 2012 to February 2015 were divided into observation group(n=42) and control group(n=25). The control group was treated with conventional drug therapy, and the observation group was treated with ultrasound ablation combined with dilation and stent implantation. The curative effect and the occurrence of the end events were observed and compared between the two groups.Results The patency rate of the observation group(85.7%) was significantly higher than that of the control group(44.0%), P<0.01. The mortality rate, the amputation rate and the incidence of cardiovascular events in the observation group were significantly lower than those in the control group(2.4% vs 24.0%, 2.4% vs 24.0%, 4.8 % vs 36.0%)(P<0.05). The ankle brachial index of the observation group(0.82±0.11) was significantly higher than that of the control group(0.61±0.12), P<0.01.Conclusion Ultrasound ablation combined with angioplasty and stent implantation is effective in the treatment of the artery occlusion in lower extremities.It can effectively clear the complete occlusion of the artery, effectively improve the lower limb blood supply, and reduce the incidence of end events, however its long-term clinical effect still need to be further observed.
