ICU复杂腹腔感染患者病原菌分析

摘要:
[摘要] 目的 探讨重症医学科(ICU)内复杂腹腔感染患者的病原菌特征。方法 回顾性分析2013-01~2016-12岑溪市人民医院ICU收治的82例确诊为复杂腹腔感染且有腹腔病原学资料的临床资料,分析腹腔细菌学分布情况。结果 (1)复杂腹腔感染病原菌的分布:革兰氏阴性菌52.7%(58/110),真菌29.1%(32/110),革兰氏阳性菌18.2%(20/110);(2)医院获得性感染组发生真菌感染的比例高于社区获得性感染组(P<0.05);(3)放弃/死亡组较存活组更容易发生真菌感染与混合细菌感染(P<0.05),同时不恰当使用抗菌药物的比例更高(P<0.05)。结论 复杂腹腔感染病原菌以革兰氏阴性菌和真菌发生率较高;腹部手术后腹腔真菌(主要是白色念珠菌)以及混合感染发生率较高,且影响预后,值得临床重视。

Abstract:
[Abstract] Objective To investigate the characteristics of pathogens in patients with complicated intra-abdominal infections in Intensive Care Unit(ICU).Methods The clinical data of 82 ICU patients with complicated intra-abdominal infections were retrospectively analyzed and the distribution of pathogenic bacteria was described.Results (1) Gram-negative bacteria were found in 58 cases(52.7%), fungi in 32 cases(29.1%) and Gram-positive bacteria in 20 cases(18.2%).(2)The proportion of fungal infections was higher in the hospital-acquired group than that in the community-acquired group(P<0.05).(3)Compared with the survival group, the death group had higher fungal infections and bactera mixed infections(P<0.05), and more cases of initial inappropriate antibiotic therapy(P<0.05).Conclusion The two most frequently isolated pathogens in the patients with complicated intra-abdominal infections are Gram-negative bacteria and fungi. Laparotomy is a risk factor for the incidence of complications such as fungal infections(mostly result from Candia albicans) and mixed infections, which affects the prognosis.
