| ［摘要］ 目的 探讨髌骨旁内侧与股内侧肌下入路行全膝关节置换术的临床效果。方法 选择兰考第一医院2014-02～2015-03收治的需全膝关节置换术的骨关节炎患者60例，根据手术方式的不同分为两组，各30例。对照组实施髌骨旁内侧入路全膝关节置换术，观察组实施股内侧肌下入路全膝关节置换术。比较两组各项手术指标和膝关节功能情况。结果 观察组置换时间较对照组长，但切口长度、下床活动时间与对照组相比有明显缩短，差异有统计学意义（P<0.01）。术前、术后1年两组患者膝关节功能对比，差异无统计学意义（P>0.05），但术后3个月观察组膝关节功能评分明显高于对照组，差异有统计学意义（P<0.05）。结论 与髌骨旁内侧入路方式相比，实施股内侧肌下入路全膝关节置换术效果更好，能减少对患者的创伤，减轻患者病痛，短期内膝关节功能恢复更优。 |

| ［Abstract］ Objective To explore the clinical effects of medial patellar approach and inferior femoral medial approach on total knee arthroplasty.Methods 60 patients with osteoarthritis were treated with total knee arthroplasty in the first hospital of Lankao County from February 2014 to March 2015 and were divided into two groups according to the different types of surgery, with 30 cases in each group. In the control group, the total knee arthroplasty was carried out by the medial patellar approach, and the total knee arthroplasty was carried out by inferior femoral medial approach in the observation group. The surgical indicators and the knee function were compared between the two groups.Results Compared with those in the control group, the replacement time was significantly longer but the length of incision and the time for ambulation were significantly shorter in the observation group(P<0.01). There was no significant difference in the knee joint function between the two groups before surgery and one year after the surgery(P>0.05), however the scores of knee joint function in the observation group were significantly higher than those in the control group 3 months after the surgery(P<0.05).Conclusion Compared with the medial patellar approach, inferior femoral medial approach is more effective on performing total knee arthroplasty in the patients with osteoarthritis. |
