多频振动治疗仪在危重症患者急性胃肠损伤早期康复治疗中的应用效果观察

摘要:
[摘要] 目的 探讨多频振动治疗仪在危重症患者急性胃肠损伤(AGI)早期康复治疗中的应用效果。方法 选取2016-09~2017-09入住该院重症医学科的70例危重症患者作为研究对象,采用随机数字法分为观察组和对照组各35例。对照组给予常规治疗,观察组在对照组基础上加用多频振动治疗仪治疗。比较两组治疗前后AGI分级、肠鸣音分级、腹腔内压力、降钙素原、C-反应蛋白、ICU住院时间、ICU住院费用及住院期间病死率情况。结果 两组治疗后3 d AGI分级比较差异无统计学意义(P>0.05),观察组治疗后7 d、28 d AGI分级均优于对照组,差异有统计学意义(P<0.05);两组治疗后3 d肠鸣音分级比较差异无统计学意义(P>0.05),观察组治疗后7 d、28 d肠鸣音分级均优于对照组,差异有统计学意义(P<0.05);两组治疗后腹腔内压力、降钙素原及C-反应蛋白均较治疗前降低(P<0.05),且观察组降低幅度均大于对照组,差异有统计学意义(P<0.05);观察组ICU住院时间短于对照组,但ICU住院费用高于对照组,差异有统计学意义(P<0.05);两组28 d病死率比较差异无统计学意义(P>0.05)。结论 危重症AGI患者早期使用多频振动治疗仪能够有效改善患者的胃肠功能,促进患者胃肠道功能的恢复,值得临床推广。

Abstract:
[Abstract] Objective To study the effect of multi-frequency vibration therapy instrument on earlier intestinal rehabilitation of acute gastrointestinal injury(AGI) in critical patients.Methods Seventy patients with AGI were enrolled in the Intensive Care Unit(ICU) of the People′s Hospital of Guangxi Zhuang Autonomous Region between September 2016 and September 2017, and were divided into observation group(n=35) and control group(n=35) by random number table method. The control group was given conventional therapy. The observation group received the same treatment as the control group plus multi-frequency vibration therapy. The AGI and bowel sound grades, the levels of intra-abdominal pressure, procalcitonin and C- reactive protein, the length of ICU stay, ICU hospitalization expenses and mortality rate during hospitalization were compared between the two groups.Results There were no significant differences between the two group in the AGI and bowel sound grades 3 days after treatment(P>0.05). The improvements of the AGI and bowel sound grades in the observation group were better than those in the control group 7 and 28 days after treatment(P<0.05). After treatment, the levels of intra-abdominal pressure, procalcitonin and C- reactive protein in both of the two groups were significantly reduced than those before treatment(P<0.05), however the reduction extent of the observation group was significantly larger than that of the control group(P<0.05). The length of ICU stay in the observation group was significantly shorter than that in the control group(P<0.05), but the ICU hospitalization expenses were higher in the observation group(P<0.05). There was no significant difference in the 28-day mortality rate during hospitalization between the two groups(P>0.05).Conclusion Early use of multi-frequency vibration therapy instrument can effectively improve the gastrointestinal function in the critical patients with AGI.
