国产超能剪在腹腔镜腹股沟疝修补术中的应用效果观察

[摘要] 目的 观察国产超能剪在腹腔镜腹股沟疝修补术中临床应用效果。方法 回顾性分析2012-07~2014-07该中心分别采用国产超能剪、进口超声刀以及电钩电剪电钳在腹腔镜腹股沟疝修补术271例的应用资料,分为三组,其中超能剪组92例,超声刀组90例,电钩电剪电钳组(简称电钳组)89例。手术方式包括完全腹膜外修补术(totally extraperitoneal,TEP)、经腹腹膜前修补术(transabdominal preperitoneal,TAPP)、腹腔内网片置入术(intraperitoneal onlay mesh,IPOM);观察记录三组不同手术方式的手术时间、术中出血量、术后住院时间、术后并发症发生率等指标。结果 超能剪组手术时间少于超声刀组、电钳组,差异有统计学意义(P<0.05);而术中出血量、术后住院时间、术后并发症发生率三组差异无统计学意义(P>0.05)。结论 国产超能剪在腹腔镜腹股沟疝修补术的临床应用是安全可行的,能缩短手术时间,部分功能优于进口超声刀、电钩、电剪及电钳。

A comparative study of super shear system in laparoscopic inguinal hernia repair

Abstract:
[Abstract] Objective To investigate the advantages and disadvantages in laparoscopic inguinal hernia repair using home-made super shear system.Methods The clinical data of two hundred and seventy-one patients with primary inguinal hernia undergoing laparoscopic inguinal hernia repair including totally extraperitoneal, transabdominal preperitoneal, intraperitoneal onlay mesh using home-made super shear system, harmonic scalpel, electrical hook, electrical shear, and electrical clip, were analyzed retrospectively between July 2012 and July 2014. The operative time, the volume of blood, hospital stay after the operation and the complications were compared among the three groups. The patients were divided into three groups, in which group 1(n=92) was subject to super shear system(TEP53, TAPP27, IPOM12); group 2(n=90) was subject to harmonic scalpel(TEP50, TAPP30, IPOM20) and group 3(n=89) was subject to electrical hook, electrical shear, and electrical clip(TEP52, TAPP26, IPOM11).Results The operative time in the group treated with super shear system was shorter than that in the other two groups(P<0.05 ). There were no significant differences in the volume of blood, hospital stay and the complications after the operation among the three groups(P>0.05).Conclusion It is safe and effective to perform laparoscopic repair of inguinal hernia using super shear system.
