| ［摘要］ 目的 探讨彩色多普勒超声诊断先天性椎动脉变异的价值和临床意义。方法 选取该院2011-01～2015-12收治的150例疑似椎动脉变异患者作为研究对象，所有患者均接受彩色多普勒超声检查，以椎动脉CT造影（CTA）结果作为诊断金标准，计算彩色多普勒超声诊断对椎动脉变异的诊断敏感性、特异性、准确性、阳性预测值、阴性预测值，并计算彩色多普勒超声诊断对先天性椎动脉发育不良、椎动脉起点位置异常、单侧双支椎动脉的诊断符合率。结果 150例疑似椎动脉变异患者中，共有131例患者经椎动脉CTA诊断为椎动脉变异。彩色多普勒超声诊断对椎动脉变异的诊断敏感性、特异性、准确性、阳性预测值、阴性预测值分别为97.71%、89.47%、96.67%、98.46%、85.00%，其对先天性椎动脉发育不良、椎动脉起点位置异常、单侧双支椎动脉的诊断符合率分别为96.72%、100.00%、100.00%。结论 彩色多普勒超声在先天性椎动脉变异的临床诊断中具有显著的应用价值。 |

| ［Abstract］ Objective To study the value and clinical significance of color Doppler ultrasound in the diagnosis of congenital vertebral artery variation.Methods One hundred and fifty patients with suspected vertebral artery variation in the hospital from January 2011 to December 2015 were selected as the research subjects. All the patients underwent color Doppler ultrasonography in vertebral artery, and their diagnostic results of CT angiography(CTA) were used as the gold standard. The diagnostic sensitivity, specificity, accuracy, positive predictive value and negative predictive value of color Doppler ultrasound in the diagnosis of vertebral artery were calculated, and the diagnosis rates of the color Doppler ultrasound in the congenital hypoplastic vertebral artery, abnormal position of vertebral artery origin and unilateral double branch vertebral artery were calculated.Results Of the 150 patients with suspected vertebral artery variation, 131 cases were diagnosed with vertebral artery variation by CTA. The sensitivity, specificity, accuracy, positive predictive value and negative predictive value in the diagnosis of vertebral artery variation were 97.71%, 89.47%, 96.67%, 98.46% and 85.00% respectively. The diagnosis rates of color Doppler ultrasound in the congenital hypoplastic vertebral artery, abnormal position of vertebral artery origin and unilateral double branch vertebral artery were 96.72%, 100.00% and 100.00% respectively.Conclusion Color Doppler ultrasound has a significant application value in the diagnosis of congenital variation of the vertebral artery. |
