| ［摘要］ 目的 比较股骨近端防旋髓内钉（PFNA）与微创内固定系统-股骨远端（LISS-DF）内固定治疗老年人股骨粗隆间骨折的临床效果。方法 选取该院收治的老年股骨粗隆间骨折患者64例，按抽签顺序分为观察组和对照组，每组32例。对照组采用LISS-DF内固定治疗，观察组采用PFNA治疗，对比两组患者手术情况及术后并发症情况。结果 观察组手术时间及负重时间较对照组明显缩短，差异有统计学意义（P<0.01）；观察组切口长度、术中出血量、术后切口引流量较对照组明显减少，差异有统计学意义（P<0.01）；两组患者骨折愈合时间及住院时间比较差异无统计学意义（P>0.05）；两组患者并发症发生率比较差异无统计学意义（P>0.05）。结论 应用PFNA治疗老年人股骨粗隆间骨折效果较为显著，可有效缩短手术时间及负重时间，且切口长度较短，有利于减少术中出血量及术后切口引流量，值得临床推广。 |

| ［Abstract］ Objective To compare the clinical effect of proximal femoral nail anti rotation intramedullary nail(PFNA) and minimally invasive internal fixation system distal femur(LISS-DF) internal fixation on the treatment of intertrochanteric fracture in the elderly.Methods 64 elderly patients with intertrochanteric fractures in our hospital were selected and divided into two groups by a draft lottery, with 32 cases in each group. The control group was treated with LISS-DF internal fixation treatment, and the observation group was treated with PFNA. The surgical results and the postoperative complications were compared between the two groups.Results The operation time and loading time in the observation group were significantly shorter than those in the control group(P<0.01); The incision length, intraoperative bleeding volume and postoperative drainage volume in the observation group were significantly reduced compared with those in the control group(P<0.01). There were no significant differences in the fracture healing time, hospitalization time and complications between the two groups(P>0.05).Conclusion PFNA is effective in the treatment of elderly femoral intertrochanteric fracture. It can effectively shorten the operation time, loading time, and incision length, and reduce the amount of bleeding and postoperative wound drainage. |
