经胸微创非体外循环室间隔缺损封堵术临床体会

摘要:
[摘要] 目的 总结经胸微创非体外循环室间隔缺损(VSD)封堵术的经验,探讨其临床价值和手术安全性。方法 选择2016-03~2016-11采用经胸非体外循环小切口封堵术治疗VSD 10例,男2例,女8例,年龄9个月~21岁,体重7~48(平均19.35)kg,患者在食道超声心动图的监测下经右心室表面送入封堵器,闭合室间隔缺损。术前经胸超声心动图(TTE)提示VSD破口直径3~8 mm。结果 封堵成功5例,出院时,无心律失常,无残余分流、瓣膜反流及房室传导阻滞,无封堵器移位、脱落及血栓形成。其余5例中转体外循环。结论 经胸微创非体外循环封堵治疗室间隔缺损安全、可行,创伤小,值得临床广泛推广和应用。

Abstract:
[Abstract] Objective To summarize the experience of minimally invasive off-pump transthoracic closure in treatment of ventricular septal defect(VSD), and to evaluate its clinical effect and safety.Methods From March 2016 to November 2016, 10 patients with VSD(2 males and 8 females, weighed 7 to 48 kg with a mean of 19.35 kg, aged 9 months to 21 years) underwent a closure of VSD through transthoracic using Amplatzer occlude from right ventricular surface. Before the operation, all the cases had been diagnosed with a 3 to 8 mm VSD by transthoracic echocardiography(TTE).Results 5 cases were cured with satisfaction. No arrhythmia, no residual shunt, no valve regurgitation, no atrioventricular block, no occlude displacement, and no embolization occurred on discharge. The other five cases were converted to cardiopulmonary bypass.Conclusion The minimally invasive off-pump transthoracic closure is safe and feasible for treating VSD, with advantages of small incision and quick recovery.
