经额部锥孔引流与小骨窗开颅术治疗高血压脑出血的临床效果比较

摘要:
[摘要] 目的 比较经额部锥孔引流与小骨窗开颅术治疗高血压脑出血(hypertensive intracerebral hemorrhage,HICH)的临床疗效。方法 选择2014-04~2016-04该院收治的HICH患者140例,按随机数字表法分为观察组和对照组,各70例。观察组实施经额部锥孔引流术治疗,对照组实施小骨窗开颅术治疗。比较两组术后认知功能、神经功能缺失情况、预后情况、肢体运动功能及并发症发生情况。结果 观察组蒙特利尔认知评估量表(MOCA)、Fugl-Meyer运动功能评分及格拉斯哥预后评分(GOS)与对照组相比均有明显提高,而美国国立卫生研究院卒中量表(NIHSS)评分低于对照组,差异有统计学意义(P<0.05);观察组术后并发症发生率低于对照组,差异有统计学意义(P<0.05)。结论 与小骨窗开颅术相比,对HICH患者实施经额部锥孔引流术效果显著,能有效改善患者肢体运动功能和认知功能,降低术后神经功能缺失程度,减少并发症发生,预后较好,值得临床推广。

Abstract:
[Abstract] Objective To compare the clinical effect between frontal hole drainage and small bone window craniotomy on the treatment of hypertensive intracerebral hemorrhage(HICH).Methods 140 HICH patients in our hospital from April 2014 to April 2016 were randomly divided into two groups, with 70 cases in each group. The observation group received the treatment of frontal hole drainage, and the control group received the treatment of small bone window craniotomy. The postoperative cognitive function, the neurological deficit and prognosis, the limb motor function and complications were compared between the two groups.Results The scores of MOCA, Fugl-Meyer and GOS in the observation group were significantly elevated than those in the control group. The NIHSS scores of the observation group were significanty lower than those in the control group(P<0.01). The incidence of complications in the observation group was significantly lower than that in the control group(P<0.05).Conclusion The frontal hole drainage is more effective than the small bone window craniotomy for the patients with HICH.
