枸橼酸咖啡因与氨茶碱治疗对早产儿神经心理发育影响的比较

摘要:
[摘要] 目的 探讨枸橼酸咖啡因与氨茶碱治疗对早产儿神经心理发育的影响。方法 收集在钦州市妇幼保健院新生儿科住院的原发性呼吸暂停早产儿共120例,随机分为咖啡因组和氨茶碱组,出院后以Gesell发育量表进行随访,比较两组住院期间的治疗情况、并发症发生率,并比较两组校正12月龄、生后24月龄Gesell量表发育商情况。结果 咖啡因组呼吸暂停消失时间、用氧时间、药物治疗时间、住院天数均短于氨茶碱组,差异有统计学意义(P<0.05);咖啡因组喂养不耐受、颅内出血发生率均低于氨茶碱组,差异有统计学意义(P<0.05)。校正12个月龄咖啡因组大运动、精细动作、适应性及个人社交能力的发育商均高于氨茶碱组,差异有统计学意义(P<0.05);生后24个月龄咖啡因组大运动、精细动作发育商高于氨茶碱组,差异有统计学意义(P<0.05)。结论 枸橼酸咖啡因较氨茶碱更有助于改善早产儿呼吸暂停症状,有利于早产儿神经心理发育。

Abstract:
[Abstract] Objective To investigate the effects of caffeine citrate and aminophylline therapy on neuropsychological development in preterm infants.Methods A total of 120 preterm infants who were diagnosed with primary apnea were randomly divided into the caffeine group(n=60) and the aminophylline group(n=60). Gesell development scale(GDS) was used to assess the neuropsychological development in the preterm infants when they were followed-up regularly after discharge. The hospital course, morbidity rate and neuropsychological development were compared between the two groups at 12 months of corrected age and at 24 months.Results The disappearance time of apnea, hospital stay, duration of drug therapy and the oxygen therapy in the caffeine group were significant shorter than those in the aminophylline group(P<0.05). The incidence rates of feeding intolerance and intracranial hemorrhage in the caffeine group were significantly lower than those in the aminophylline group(P<0.05). The developmtntal quotient(DQ) scores of gross motor, fine motor, adaptability and social communicative abilities in the caffeine group were significantly higher than those in the aminophylline group at 12 months of corrected age. However, the DQ scores of gross motor and fine motor in the caffeine group were significantly higher than those in the aminophylline group at 24 months.Conclusion Caffeine citrate therapy is more conducive to the improvement of primary apnea and the neuropsychological development in premature infants.
