[摘要] 目的 探讨大剂量阿托伐他汀辅助治疗对高血压患者颈动脉内膜中层厚度(IMT)的影响。方法 将并发有颈动脉斑块的原发性高血压患者125例随机分为两组。治疗组62例,对患者实施常规降压治疗,并辅助给予大剂量阿托伐他汀40 mg/d。对照组63例进行常规降压治疗,并予阿托伐他汀10 mg/d。在治疗前、治疗6个月后对患者进行动态监测,并观察舒张压(DBP)、收缩压(SBP)、三酰甘油(TG)、血总胆固醇(TC)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)、颈动脉IMT以及颈动脉斑块的变化。结果 经治疗后,治疗组和对照组LDL-C、TC、TG和颈动脉IMT值均下降,但治疗组下降的幅度较对照组更大(P<0.01)。两组经治疗后,斑块的厚度和长径都有所减小,但治疗组较对照组更为明显(P<0.01)。结论 大剂量阿托伐他汀对高血压患者颈动脉IMT有减退作用,能使高血压患者的血脂降低,抑制高血压患者的动脉粥样硬化。 |

[Abstract] Objective To evaluate the effect of high-dose atorvastatin on carotid intima-media thickness(IMT) in hypertensive patients.Methods 125 patients with primary hypertension complicated with carotid plaques were randomly divided into the treatment group and the control group. The treatment group(n=62) received routine antihypertensive therapy plus high dose of atorvastatin 40 mg/d, and the control group(n=63) received routine antihypertensive therapy plus atorvastatin 10 mg/d. The patients were dynamically monitored before and 6 months after treatment. The changes of diastolic blood pressure, systolic blood pressure, triglyceride(TG), serum total cholesterol(TC), low density lipoprotein cholesterol(LDL-C), high density lipoprotein cholesterol(HDL-C) and carotid IMT were observed.Results After treatment, the levels of LDL-C, TC and TG in the treatment group and the control group decreased, but the rate of decline in the treatment group was greater than that in the control group(P<0.01). After treatment, the thickness and length of carotid IMT were reduced in both of the two groups, however the reduced values in the treatment group was greater than those in the control group(P<0.01).Conclusion Large doses of atorvastatin have a significant effect on reducing carotid IMT and blood lipids, and inhibiting atherosclerosis in hypertensive patients. |
