| ［摘要］ 目的 探讨硬膜外阻滞麻醉无痛分娩对初产妇产程及新生儿结局预后的影响。方法 选取2015-08～2016-05在该院进行分娩的产妇90例作为研究对象。将所有产妇按随机数字表法分为两组，各45例。对照组不采用任何镇痛措施，观察组采用硬膜外阻滞麻醉无痛分娩方式，对比两组产妇镇痛效果、产程进展及新生儿评分。结果 观察组产妇镇痛效果明显优于对照组，差异有统计学意义(P<0.01)；观察组产妇第一及第二产程时间均较对照组短，新生儿Apgar评分高于对照组，差异有统计学意义(P<0.01)；两组产妇第三产程时间比较差异无统计学意义(P>0.05)。结论 硬膜外阻滞麻醉无痛分娩可有效缩短初产妇的产程，降低新生儿宫内缺氧或窒息的发生率，提高Apgar评分，改善分娩结局，安全性较高，值得在临床中应用推广。 |

| ［Abstract］ Objective To investigate the effect of painless labor caused by epidural anesthesia on the duration of labor in primiparas and the prognosis of the neonatal outcome.Methods 90 primiparas receiving delivery in our hospital from August 2015 to May 2016 were selected as the research subjects and were randomly divided into two groups, with 45 cases in each group. The control group did not use any analgesic measures, while the observation group received epidural anesthesia for painless delivery. The analgesic effect, the duration of labor and the Apgar scores of neonates were compared between the two groups.Results The analgesic effect of the observation group was significantly better than that of the control group(P<0.01). The time for the first and the second stages of labor in the observation group was shorter than that in the control group. The Apgar scores of the observation group were higher than those of the control group(P<0.01). There was no significnthy difference in the third labor time between the two groups(P>0.05).Conclusion Epidural anesthesia is safe and effectively shortens the duration of labor, reduces the neonatal asphyxia or intrauterine hypoxia and improves the Apgar scores and the birth outcomes for the primiparas. |
