无创呼吸机辅助呼吸治疗急性心力衰竭的疗效观察

摘要:
[摘要] 目的 观察无创呼吸机通气治疗急性心力衰竭的疗效。方法 选取2015-03~2017-03以“急性心力衰竭”就诊的患者100例,随机分为对照组和治疗组各50例。对照组予以吸氧、镇静、扩血管、利尿、维持电解质平衡及纠正酸碱紊乱等常规基础治疗;治疗组在对照组治疗的基础上早期予以无创呼吸机通气治疗。观察记录两组患者治疗前后的有效率、气管插管率,呼吸频率(RR)、心率(HR)及血气水平[氧分压(PaO2)、二氧化碳分压(PaCO2)],并进行比较分析。结果 治疗组治疗有效率明显高于对照组(P<0.05);治疗组气管插管率明显低于对照组,差异有统计学意义(P<0.05);治疗组患者的HR、RR、PaO2、PaCO2值较对照组明显改善,差异有统计学意义(P<0.05)。结论 在常规治疗的基础上早期予以无创呼吸机通气治疗急性心力衰竭能明显提高临床治疗的有效率,降低气管插管率,尽快改善患者症状,值得临床推广应用。

Abstract:
[Abstract] Objective To evaluate the efficacy of noninvasive ventilation in treatment of acute heart failure.Methods From March 2015 to March 2017, 100 patients with acute heart failure were selected in our hospital, and were randomly divided into the control group and the treatment group,with 50 cases in each group. The control group received the routine treatment, including oxygen inhalation, sedation, vasodilator, diuretic, maintaining electrolyte balance and correction of acid and alkali disorders. The treatment group received early noninvasive ventilation therapy plus the same routine treatment as the control group. The clinical efficacy, the rate of endotracheal intubation, respiratory rate(RR), heart rate(HR) and blood gas levels(PaO2, PaCO2) were compared between the two groups.Results After treatment, the effective rate of the treatment group was significantly higher than that of the control group(P<0.05), while the rate of endotracheal intubation in the treatment group was significantly lower than that in the control group(P<0.05). The improvements of HR, RR, and the levels of PaO2 and PaCO2 in the treatment group were better than those in the control group(P<0.05).Conclusion Early noninvasive ventilation therapy plus the routine treatment can significantly improve the clinical efficiency, reduce the rate of endotracheal intubation and improve the symptoms in the patients with acute heart failure.
