预先硬膜外腔置管镇痛对分娩效果与方式的影响

摘要:
[摘要] 目的 观察硬膜外腔预先置管对分娩镇痛效果与分娩方式的影响。方法 选择有阴道试产条件且要求硬膜外腔分娩镇痛产妇120例,分A、B两组,各60例。A组:有规律宫缩,宫口<3 cm,进行硬膜外腔置管,在产妇无法忍受疼痛时给药镇痛。B组:有规律宫缩,宫口≥3 cm,进行硬膜外腔置管并即刻给药镇痛。结果 A、B两组产妇分娩镇痛效果好,两组比较差异无统计学意义(P>0.05),但是由于A组对镇痛时机有个体化的主动权,对整个分娩期间的镇痛满意度更高,与B组比较差异有统计学意义(P<0.05)。两组缩宫素使用率、中转剖宫产率、第一产程时间、产后2 h出血量、新生儿1 min、5 min Apgar评分等指标比较差异无统计学意义(P>0.05)。结论 硬膜外腔预先置管分娩镇痛效果可靠、满意度更高、母婴安全、不增加中转剖宫产率。

Effects of advanced epidural cavity catheter analgesia on delivery modes and outcomes

Abstract:
[Abstract] Objective To observe the effects of advanced epidural cavity catheter analgesia on delivery modes and outcomes.Methods 120 parturient women with epidural labor were divided into group A and group B, with 60 cases in each group. In group A, there were regular contractions, orifice of the uterus <3 cm, and the epidural catheter was used to relieve pain when the women were unable to endure delivery pain. In group B, there were regular contractions, orifice of the uterus ≥3 cm, and the epidural catheter was inserted into the catheter and analgesia was immediately administered. The effects of analgesia were compared between the two groups. Results Both of group A and group B had good delivery analgesia effects, and there were no significant difference between them(P>0.05). However, group A had more initiative in the timing of analgesia than group B, and the satisfaction degree of delivery anesthesia in group A was higher than that in group B(P<0.05). There were no significant differences between the two groups in the rate of using oxytocin, the rate of transferring to caesarean section, the time for the first stage of labor, the blooding volume 2 hours after delivery, and the Apgar scores of the newborns in 1 minute (P>0.05).Conclusion The analgesic effect of epidural catheterization is safe, reliable and satisfying for the mothers and their newborns.
