不同降脂强度对卒中后患者神经功能缺损血脂水平 颈动脉内-中膜厚度的影响

摘要:
[摘要] 目的 探讨他汀不同降脂强度对卒中后患者神经功能缺损、血脂水平、颈动脉内-中膜厚度的影响。方法 选取该院住院的脑梗死(卒中)患者68例,随机分为研究组和对照组各34例。在常规治疗基础上,研究组给予阿托伐他汀钙40 mg/次,1次/d,疗程3个月;对照组给予阿托伐他汀钙20 mg/次,1次/d,疗程3个月。分别于治疗前、治疗后3个月进行美国国立卫生研究院卒中量表(NIHSS)评分,收集血脂数据,应用彩色多普勒超声诊断仪检测颈动脉内-中膜厚度。结果 两组治疗前、治疗后3个月NIHSS评分比较差异无统计学意义(P>0.05)。治疗后,研究组总胆固醇(TC)、低密度脂蛋白胆固醇(LDL-C)及颈动脉内-中膜厚度较对照组显著降低(P<0.05);而甘油三酯(TG)、高密度脂蛋白胆固醇(HDL-C)两组间未见显著差异。结论 相较于中强度的阿托伐他汀(20 mg),高强度的阿托伐他汀(40 mg)可显著降低TC及LDL-C水平,稳定颈内动脉斑块,减小颈动脉内-中膜厚度。

Abstract:
[Abstract] Objective To explore the effects of different lipid-decreasing intensities on poststroke nerve function defect, blood lipid level and intima-media thickness of carotid artery.Methods 68 patients with acute cerebral infarction were randomly divided into the research group(n=34) and the control group(n=34). The research group was treated with atorvastatin calcium(40 mg, qd) for 3 months.The control group received calcium atorvastatin(20 mg, qd) for 3 months. NIHSS scores and the data of serum lipid level were collected before and after treatment. Intima-media thickness of carotid artery was detected by TCD method.Results There were no significant differences in NIHSS scores between the research group and the control group before and 3 months after treatment. After treatment, TC, LDL-C and the intima-media thickness of carotid artery in the research group were significantly lower than those in the control group. However, there were no significant differences in TG and HDL-C between the two groups.Conclusion Large dose of atorvastatin(40 mg) can decrease TC and LDL-C, stabilize internal carotid artery plaque, and reduce intima-media thickness of carotid artery on a larger scale than small dose of atorvastatin(20 mg).
