| ［摘要］ 目的 探讨神经内镜经鼻蝶窦入路切除垂体瘤的临床效果。方法 对2015-01～2016-01该院使用神经内镜经鼻蝶窦入路进行切除的19例垂体瘤患者的临床效果进行观察。在手术治疗前和治疗后1个月对患者进行美国国立卫生研究所脑卒中评分表（NIHSS）评分，同时观察患者的并发症发生情况。结果 在手术治疗后1个月患者的NIHSS评分为（16±5）分，高于手术治疗前的（11±2）分，差异有统计学意义（P<0.05）。该组患者共发生5例尿崩症（其中4例一过性尿崩症，1例临时性尿崩症），2例脑脊液鼻漏，3例电解质紊乱，1例垂体分泌功能低下。在采取相应的措施治疗后，并发症全部好转，无一例死亡。结论 在手术的过程中严格按照操作规范进行操作，术后加强并发症防治措施，对神经内镜经鼻蝶窦入路切除垂体瘤术的预后具有重要意义。 |

| ［Abstract］ Objective To explore the clinical effect of endoscopic resection of pituitary tumor by endoscopic sinus approach. Methods From January 2015 to January 2016, the clinical effect of 19 patients with endoscopic sinus approach in our hospital were observed. Before surgery and 1 month after the treatment, NIHSS scores of the patients were recorded and their complications were observed. Results One month after the surgery, the NIHSS scores of the patients(16±5) were significanthy higher than those before the operation(11±2)(P<0.05). A total of 5 cases occurred diabetes insipidus, including 4 cases of transient diabetes insipidus and 1 case of temporary diabetes insipidus. 2 cases occurred cerebrospinal rhinorrhea. 3 cases occurred electrolyte disorder and 1 case occurred pituitary secretion dysfunction. After the corresponding measures were taken, the complications were all improved, and there were no dead cases. Conclusion During the operation of pituitary tumor resection by endoscopic sinus approach, it is clincally significant to follow the operation norms strictly, and to strengthen the prevention and treatment of postoperative complications. |
