支气管肺泡灌洗液肿瘤标记物检测对鉴别周围型典型孤立肺结节性质的临床意义

摘要:
[摘要] 目的 探讨支气管肺泡灌洗液(BALF)肿瘤标记物检测对周围型典型孤立肺结节的诊断价值。方法 纳入2015-01~2016-01周围型孤立肺结节患者156例,术后病理确诊肺癌126例(腺癌123例,鳞癌2例,小细胞肺癌1例),良性病变30例(炎性假瘤7例,错构瘤12例,血管瘤8例,炎性肉芽肿3例)。分为肺恶性结节组和良性病变组。对两组患者支气管肺泡灌洗液癌胚抗原(CEA)、细胞角质蛋白21-1片段(CYFRA21-1)、神经源性烯醇化酶(NSE)及鳞状细胞癌相关抗原(SCCA)含量进行检测及比较。结果 肺恶性结节组BALF中CEA及CYFRA21-1含量明显高于良性病变组(P<0.05),而NSE及SCCA含量无明显差异(P>0.05)。结论 BALF中CEA及CYFRA21-1对预判周围型典型孤立肺结节的性质有较高的价值,而NSE及SCCA价值有限。

Abstract:
[Abstract] Objective To evaluate the diagnostic value of bronchoalveolar lavage fluid(BALF) tumor marker detection in differentiating the nature of peripheral typical isolated pulmonary nodules. Methods 156 patients were included in this study who were pathologically diagnosed with solitary pulmonary nodules in our hospital from January 2015 to January 2016,including 126 cases of lung cancer(123 cases of adenocarcinoma, 2 cases of squamous carcinoma and 1 case of small cell lung cancer), 30 cases of benign lesions(7 cases of inflammatory pseudotumor, 12 cases of hamartoma, 8 cases of hemangioma and 3 cases of inflammatory granuloma). The contents of carcino-embryonic antigen(CEA), cytokeratin 19 fragment(CYFRA21-1), neurone specific enolase(NSE) and squamous cell carcinoma antigen(SCCA) in the BALF were detected and compared between the two groups. Results The contents of CEA and CYFRA21-1 in the lung cancer group were significantly higher than those in the benign group(P<0.05).There were no significant differences in the contents of NSE and SCCA between the two groups(P>0.05). Conclusion CEA and CYFRA21-1 in BALF have high value in diagnosing peripheral solitary pulmonary nodules, while NSE and SCCA are of limited value.
