[摘要] 目的 通过X线头影测量方法评估安氏Ⅰ类错(牙合)畸形早期恒牙(牙合)正畸治疗拔牙与非拔牙矫治方式对上气道形态及舌骨位置的影响,进而指导临床矫治的实施。方法 收集安氏Ⅰ类错(牙合)畸形早期恒牙(牙合)患者60例,以治疗方式的不同将其分为拔牙组与非拔牙组,每组30例,对其上气道形态及舌骨位置进行测量、对比分析。结果 非拔牙组组内治疗前后比较,PNS-UPW、U-MPW、H-FH、H-VL测量项目差异有统计学意义(P<0.05);拔牙组组内治疗前后比较,PNS-UPW、SPP-SPPW、U-MPW、H-MP测量项目差异有统计学意义(P<0.05);非拔牙组与拔牙组治疗后PNS-UPW测量项目比较差异有统计学意义(P<0.05),其余测量项目差异无统计学意义(P>0.05)。结论 安氏Ⅰ类早期恒牙(牙合)患者正畸拔牙矫治对上气道形态的变化无明显影响,对舌骨位置无明显影响,但矢状方向上有产生向后向下的移动趋势。 |

[Abstract] Objective To evaluate the effects of premolar extraction and non-tooth extraction correction on upper airway morphology and hyoid position in immature premature permanent occlusion of Anglel ClassⅠ maloocclusion by cephalometrics to guide the implementation of clinical correction. Methods Sixty patients with immature premature permanent occlusion of Anglel ClassⅠ maloocclusion were seleted and the patients were divided into premolar extraction group and non-tooth group according to their different therapy methods, with 30 cases in each group. The changes of upper airway morphology and hyoid position were observed before and after the extraction treatment. Results The differences in PNS-UPW, U-MPW, H-FH and H-VL measurement items were statistically significant in the non-tooth extraction group before and after treatment(P<0.05). The measurement items of PNS-UPW, SPP-SPPW, U-MPW and H-MP were significantly different in the premolar extraction group before and after the extraction treatment(P<0.05).There was a statistically significant difference in the measurement item of PNS-UPW between the non-tooth extraction and the premolar extraction group after treatment(P<0.05). There were no statistically significant differences in the other measurement items between the two groups(P>0.05). Conclusion Angle ClassⅠ with premature permanent occlusion has no significant effect on the change in the upper airway morphology and has no obvious effect on hyoid position, but produces backward on hyoid position in the sagittal direction and has a downward movement trend. |
