应用床旁腔内超声诊断极低出生体重儿脑室周围-脑室内出血的临床意义

摘要:
目的 探讨床旁腔内超声在极低出生体重儿脑室周围-脑室内出血(periventricular-intraventriculaLr hemordlage,PVH-IVH)的应用价值。方法 选择2009年5月至2014年5月在我院新生儿重症监护室(NICU)住院的179例早产极低出生体重儿,行床旁经阴道超声探头(腔内超声)经颅检查,根据Papile分级法将早产儿PVH-IVH分为Ⅰ、Ⅱ、Ⅲ、Ⅳ级。结果 1. 179例极低出生体重儿超声诊断PVH-IVH共45例(发生率25.1%)。2. PVH-IVH发生率的比较,不同胎龄和不同出生体重的早产儿,差异有统计学意义(P<0.05)。3. PVH-IVH出血严重程度的比较,不同胎龄和不同出生体重的早产儿,差异无统计学意义(P>0.05)。4.与头颅MRI比较,经阴道超声探头灵敏度和特异性均为100%,腹部超声头灵敏度为100%,特异性为94.74%。结论 胎龄越小、出生体重越轻,PVH-IVH发生率越高。床旁腔内超声对早产儿PVH-IVH的诊断有较高价值。

Abstract:
Objectives?This article studies applied values of?periventricular-intraventricular hemorrhage?(PVH-IVH) through?bedside intraluminal ultrasound in Very Low Birth Weight Infants (VLBWI)(The birth weight is less than 1500g).Method One hundred and seventy nine (179) VLBWIs who are admitted to NICU of my hospital from May 2009 through May 2015 are included in this study. Bedside transvaginal ultrasound is performed for transcranial check on these infants.?Based on Papile classification, premature infants with?PVH-IVH are serially graded?as Grade I, Grade?Ⅱ,?Grade?Ⅲ?and?Grade?Ⅳ. Results1. Forty five (45) VLBWIs are diagnosed with?PVH-IVH by ultrasound from the one hundred and seventy nine (179) VLBWIs. Occurrence is 25.1%. 2. By comparing PVH-IVH occurrence, there are significant statistical differences (P﹤0.05) in premature infants in terms of different embryonic ages and birth weights. 3. There are significant statistical differences (P﹥0.05) in premature infants in terms of different embryonic ages and birth weights, after?comparing?PVH-IVH?severity. 4.?By comparing cerebral MRI, transvaginal ultrasound probes are 100% sensitive and specific. Abdominal ultrasound probes are 100% sensitive and 94.74% specific.Conclusion The smaller the?embryonic age and birth weight are, the higher PVH-IVH occurrence will be.?Bedside intraluminal ultrasound is of higher value for diagnosing?PVH-IVH in premature infants.
