BALF肿瘤标记物检测对鉴别周围型典型孤立肺结节性质的临床意义

摘要:目的 探讨支气管肺泡灌洗液肿瘤标记物检测于周围型典型孤立肺结节的诊断价值。方法 纳入2015年1月至2016年1月周围型孤立肺结节患者156例,术后病理确诊肺癌126例(腺癌123例,鳞癌2例,小细胞肺癌1例),良性病变30例(炎性假瘤7例,错构瘤12例,血管瘤8例,炎性肉芽肿3例)。对两组患者灌洗液CEA(癌胚抗原)、CYFRA21-1(细胞角质蛋白21-1片段)、NSE(神经源性烯醇化酶)及SCC(鳞状细胞癌相关抗原)含量进行检测及比较。结果 肺癌组支气管肺泡灌洗液CEA及CYFRA21-1含量明显高于良性组(P<0.05),而NSE及SCC含量无明显差异(P>0.05)。结论支气管肺泡灌洗液CEA及CYFRA21-1对预判周围型典型孤立肺结节的性质有较高的价值,而NSE及SCC价值有限。

Abstract:
Abstract: Objective To evaluate the diagnostic value of bronchoalveolar lavage tumor markers in peripheral typical isolated pulmonary nodules. Methods We included 156 cases of solitary pulmonary nodules in our hospital from January 2015 to January 2016,postoperative pathological diagnosis of lung cancer in 126 cases (123 cases of adenocarcinoma, 2 cases were squamous carcinoma, small cell lung cancer 1 case), 30 cases of benign lesions (12 cases, 7 cases of inflammatory pseudotumor hamartoma, hemangioma, 8 cases of inflammatory granuloma in 3).The contents of CEA, CYFRA21-1, NSE and SCC in the two groups were detected and compared. Results The content of CEA and cyfra21-1 in bronchus lavage in lung cancer group was significantly higher than that in the benign group (P<0.05),There was no significant difference between NSE and SCC(P>0.05).Conclusions The value of CEA and CYFRA21-1 in the predetermination of the typical isolated lung nodules in the surrounding type is of high value, while NSE and SCC are of limited value.
