血清与脑脊液PCT、NSE检测在小儿颅内感染中的诊断价值

摘要:
【】 目的 观察血清和脑脊液中降钙素原(PCT)、神经元特异性烯醇化酶 (NSE )含量变化及在颅内感染患儿中的临床诊断价值。 方法 选取62例颅内感染患儿,30例化脓性脑膜炎 (化脑组),32例病毒性脑炎(病脑组)和33例对照组,测定血清及脑脊液中PCT、 NSE含量,并分析其与脑脊液糖、蛋白指标的相关关系。 结果 化脑组血清与脑脊液中PCT均高于病脑组和对照组,差异有统计学意义 (P<0.05);病脑组血清与脑脊液NSE含量显著高于化脑组和对照组,差异有统计学意义(P<0.05)。化脑组脑脊液糖降低、蛋白水平升高与病脑组及对照组对比,差异有统计学意义(P<0.05)。血清PCT、NSE与脑脊液中PCT、NSE含量均呈极强正相关 (P<0.001 )。血和脑脊液PCT与脑脊液蛋白 、糖均呈强相关性(P<0.001)。结论 血清和脑脊液PCT、NSE含量变化均有助于颅内感染类型及脑损伤程度的判断。检测血清PCT和NSE的含量较脑脊液检测更加简单易操作、快捷,值得在临床推广应用。

Abstract:
Objective To observe the changes in the content of calcitonin (PCT) and neuron specific enolase (NSE) in serum and cerebrospinal fluid (CSF) and the value of clinical diagnosis in children with intracranial infection. Methods 62 cases of intracranial infection, 30 cases of purulent meningitis (PM group), 32 cases of viral encephalitis (VE group) and 33 control groups were studied. And the level of PCT and NSE in serum and CSF, and the index of sugar and protein in cerebrospinal fluid were measured. Results The levels of serum and CSF PCT in the PM group were higher than those of the VE group and the control group (P<0.05). The levels of serum and CSF NSE in the VM group were significantly higher than those of the PM group and control group (P<0.05).In group PM, the decrease of sugar and the increase of protein level in CSF were significantly different from those in the VM group and control group (P<0.05).Serum PCT and NSE were positively correlated with PCT and NSE contents in CSF (P<0.001).PCT in serum and CSF was strongly correlated with protein and sugar in CSF (P<0.001). Conclusion The changes of PCT and NSE levels in serum and CSF maybe helpful to judge the type of intracranial infection and the degree of brain injury. The detection of PCT and NSE in serum was easier to operate and works faster, and was worthy of clinical application.
