非哺乳期乳腺炎性肿块的超声图像及造影特征探讨分析

摘要:
目的:观察非哺乳期妇女乳腺炎性肿块的超声图像及超声造影表现,探讨其超声图像及超声造影的特征。方法:对广西壮族自治区人民医院2008年1月-2016年12月的3504例进行了超声造影检查的乳腺肿块患者,其中有手术及病理证实的52例非哺乳期乳腺炎性肿块患者的超声图像特征、血流情况进行观察及实时超声造影,选用SonoVue超声造影剂,对肿块的造影增强形态、强度及时间-强度曲线特征进行了观察,收集其影像学资料,分析超声及超声造影表现并与病理进行了对照分析。结果:经病理证实52例非哺乳期乳腺炎性肿块,高频彩色超声图像具有一定的表现及特征,表现为不规则实质低回声、混合型肿块,位置常较表浅,并与皮肤层分界不清,表面皮肤水肿增厚或层次模糊等,肿块常有类似恶性肿瘤的表现;超声造影特征表现为:强度常为非均质高增强,进入肿块的范围常较二维超声图像的范围明显增大,周围及内部有较粗大的条状血管增强。最具有特征性表现是丰富的造影剂进入后呈分隔状,内可见单个及多个无增强区,呈圆及椭圆状。彩色高频超声诊断符合率为58%(30/52),结合超声造影检查后诊断的符合率为83%(43/52),超声(US-BI—RADS)分级为III-V级。结论:非哺乳期乳腺炎性肿块高频彩色超声及超声造影可呈现一定的特征表现,高频彩色超声图像与超声造影两者结合有助于减少误诊及漏诊率,有助于提高非哺乳期乳腺炎性肿块的诊断符合率。

Analysis of conventional ultrasound and contrast-enhanced ultrasound images of breast masses in non-lactational mastitis

Abstract:
Objective To observed and investigated the characteristics of the conventional ultrasound and contrast-enhanced ultrasound(CEUS) images of inflammation breast masses in non-lactating female. Methods The 3504 patients with breast masses were measured by conventional ultrasound and CEUS in the People"s Hospital of Guangxi Zhuang Autonomous Region from January 2008 to December 2016, Include 52 patients with non-lactational mastitis were confirmed as benign inflammation breast masses by biopsy/surgical pathology, were observed the characteristics of the images of conventional ultrasound, blood flow and CEUS using Sono Vue. The enhancement form, intensity and time-intensity curve were observed, and evaluation of characteristics of contrast-enhanced ultrasonography and histopathology data analysis. Results 52 patients with breast masses in the non-lactational mastitis were confirmed by pathology, have characteristics high frequency color ultrasound images, all the masses had hypo and heterogenous mixed echo, the mass location is superficial and unclear boundary of the skin, the surface skin edema thickened or blurred, some masses often have similar performance to the malignant tumors. The masses have characteristics CEUS images too, Inflammatory breast masses more often showed heterogeneous high enhancement, the size of masses at contrast-enhanced ultrasound examination appeared larger than conventional ultrasound examination, there were thick vessels around and inside. The most characteristics of the CEUS images were performed the rich contrast agent entranced the messes then became separated into many areas (figure 5), there are single and multiple round or oval areas without enhanced. The accuracy of high frequency color ultrasonography was 58%(30/52), the accuracy of with contrast-enhanced ultrasonography was 83%(43/52), US-BI-RADS classified as III-V. Conclusion: The with breast masses in the non-lactational mastitis have characteristics of the conventional ultrasound and (CEUS) images, the combination of high frequency ultrasound and CEUS has the potential to reduce misdiagnosis rate, and increase the diagnostic accuracy.
