开闭口式硅橡胶取模对三类牙槽嵴全口义齿修复边缘适合性的对比研究

摘要:
目的:研究三类牙槽嵴患者制取开闭口式硅橡胶印模后全口义齿修复的边缘适合性。方法:选取2016年1月至2018年1月在我院修复科就诊的全口牙列缺失患者,按照Atwood分级法将其分级,其中二级牙槽嵴18例,三级牙槽嵴20例, 四级牙槽嵴12例。将这三类牙槽嵴又分别分为实验组和对照组,实验组患者采用闭口式硅橡胶印模行全口义齿修复,对照组患者采用开口式硅橡胶印模行全口义齿修复。修复后一个月内,统计患者因边缘不合适来诊的次数、调磨的区数及各区调磨过的次数。结果:全口义齿修复一个月内,三类牙槽嵴中实验组调磨的次数均要明显少于对照组(p<0.05),尤其是四级牙槽嵴(p<0.01);实验组三级和四级牙槽嵴调磨的区数要明显少于对照组(p< 0.05),实验组二级牙槽嵴调磨的区数和对照组没有明显的差异(p>0.05);其中下颌磨牙区边缘调磨的次数为最多,其次为上颌磨牙区。结论:闭口式硅橡胶取模制作的全口义齿边缘适合性要明显高于开口式硅橡胶取模,而且在牙槽嵴条件越差的时候效果越明显。

A comparative study on the edge suitability of complete denture about three types of alveolar ridge with open or closed-mouth silicone rubber impression

Abstract:
[Abstract] Objective: To study the edge suitability of the complete denture of the three kinds of alveolar ridge patients with open or closed-mouth silicone rubber impression. Methods: Choose patients who had edentulous between January 2016 and January 2018 in our hospital and will do the complete denture repair, classified these patients according to Atwood taxonomy, including 18 patients with level 2 alveolar ridge, 20 patients with level 3 alveolar ridge, 12 patients with level 4 alveolar ridge. Each of the three kinds of alveolar ridge were divided into experimental group and control group, experimental group were treated by complete denture repair with closed silicone rubber impression, the control group were treated by complete denture repair with open type silicone rubber impression. During one month after repaired, the number of patient review times due to inappropriate denture edges, the number of areas and the number have grinded in each district were statistically analyzed. Results: During one month after repaired, the number of adjustments of the experimental group in the three types of alveolar ridge was significantly less than that in the control group (p< 0.05), especially the level 4 alveolar ridge (p< 0.01). In the experimental group, the number of grinding areas in the level 3 and level 4 alveolar ridge were significantly less than that in the control group (p< 0.05), and there was no significant difference in grinding numbers of level 2 alveolar ridge between the experimental group and the control group (p>0.05). In the mandibular molar area, the number of grinding is the most, followed by the maxillary molar area. Conclusion: The edge suitability of the complete denture by the closed-mouth silicone rubber impression is obviously higher than that by open silicone rubber impression, and the effect is more obvious when the condition of alveolar ridge is worse.
