肺腺癌患者EGFR突变状态与CT影像学及临床特征的相关性研究

摘要:
目的:分析肺腺癌EGFR突变状态与CT影像学及临床特征的相关性。方法:收集2016年6月至2017年12月就诊于我院并行EGFR基因检测的肺腺癌患者CT影像学及临床资料。 共149例,突变80例,野生型69例。运用SPSS22.0软件对EGFR突变组与野生型组的CT影像学及临床特征行单、多因素分析。结果:CT影像学:肿瘤伴有毛刺、胸腔积液的患者EGFR突变率高(P<0.05)。临床特征:女性、无吸烟史、CEA升高的患者EGFR突变率高(P<0.05)。 Logistic回归结果提示:性别、CEA、毛刺对EGFR突变有预测价值。结论:单因素分析结果提示EGFR突变状态与性别、吸烟史、CEA、毛刺及胸腔积液有相关性。 女性、CEA升高、毛刺是EGFR突变危险因素。

Abstract:
Objective:To analyze the correlation between EGFR mutation status with chest CT imaging features and clinical features in lung adenocarcinoma patients ..Methods: Clinical data and CT imaging data from patients with lung adenocarcinoma who underwent EGFR gene testing were collected in our hospital from 2016-6 to 2017-12. A total of 149 cases, 80 cases of mutation, 69 cases of wild type.SPSS 22.0 software was used for analyzing CT features and clinical characteristics of lung cancer EGFR mutations and wild group by univariate and multivariate analysis Results:Among CT imaging characteristics, the mutation rate of EGFR in patients with tumor accompanied by burr and pleural effusion was higher (P<0.05). Among clinical features, the proportion of women with no smoking history and elevated tumor marker CEA were higher in the EGFR mutation group than in the EGFR-free group (P<0.05).Logistic regression analysis showed that gender, tumor marker CEA, and CT imaging have predictive value for EGFR mutations in lung adenocarcinoma. Conclusion: Univariate analysis results suggest EGFR mutation status of lung adenocarcinoma is related to the patient's gender, smoking history, tumor marker CEA, burr sign and pleural effusion. Logistic regression models suggest that women, elevated CEA, and burr signs are risk factors for EGFR mutations in patients with lung adenocarcinoma.
