［Abstract］　With the rapid development of organ transplantation technology, liver transplantation has become an effective method for the treatment of end-stage liver diseases. However, the shortage of donor livers limits the further development of liver transplantation. In practical clinical applications, marginal donors and expanded standard donors have gradually been widely used to alleviate the contradiction between supply and demand of the organs. This article elaborates on the experience of organ evaluation, maintenance and acquisition after the death of citizens in the First Affiliated Hospital of Zhengzhou University in recent years to provide reference for timely, effective and rapid assessment of the donor livers, and to ensure the survival rate and safety of the transplant recipients.

https://www.zglcxyxzz.com/zglcxyyen/ch/reader/view_abstract.aspx?file_no=20191003&flag=1

［Abstract］　Objective　To evaluate the efficacy and safety of basiliximab combined with anti-T-lymphocyte globulin-Fresenius(ATG-F) as immunity induction of renal transplantation. Methods　The clinical data of the patients who received the first kidney transplantation in Nanfang Hospital from January 1, 2014 to December 31, 2014 were retrospectively analyzed. The recipients receiving basiliximab combined with ATG-F induction were included as the observation group, and the recipients of contralateral donor kidney receiving single ATG-F induction were included as the control group. Fourteen pairs of the patients were included in the two groups and all the patients underwent the triple maintenance therapy of immunosuppression(tacrolimus+mycophenolate mofetil+Medral). The differences in the incidence rates of delayed renal function recovery(DGF), acute rejection(AR), pulmonary infection and secondary diabetes, renal function and the trough value of tacrolimus(FK-506) in blood within 1 year after kidney transplantation were compared between the two groups. Results　There were no graft loss and death cases in both groups during the first year after renal transplantation. The incidence rates of AR and DGF in the observation group and the control group were ［14.29%(2/14) vs 7.14%(1/14)］ and ［14.29%(2/14) vs 14.29%(2/14)］ respectively. There were no secondary diabetes in the observation group and 1 case in the control group, with no significant difference between the two groups(P>0.05). There was no significant difference in the incidence of pulmonary infection within 1 year after kidney transplantation between the observation group(14.29%) and the control group(21.43%)(P>0.05). The trough value of FK-506 in the observation group was significantly lower than that in the control group on the 3rd, 6th and 9th month after operation(P<0.05). Conclusion　Basiliximab combined with ATG-F induction can effectively prevent rejection after renal transplantion without increasing the incidence of infectious complications, and reduce the exposure of calcineurin inhibitors(CNIs) drugs in early renal transpation patients.

［Abstract］　Objective　To survey the quality of life(QOL) among the pediatric patients with liver transplantation for 5 years. Methods　The Pediatric Quality of LifeTM 3.0 Transplant Module was adopted to develop the questionnaire of the survey. Forty pediatric patients who had received liver transplantation for 5 years were investigated. Results　The median age at the time of transplantation was 11 months, and the median age at the time of the survey was 97.5 months. The median height of the pediatric patients was 129.5(110,168)cm. The median weight was 29.3(17.0, 88.5)kg, and the median body mass index(BMI) was 15.6(10.7, 31.4)kg/m2. The median weight Z value was 0(-1.97, 2.18) and the median height Z value was 0.38(-2.48, 2.53). The median BMI Z value was -0.26(-4.47, 3.16). The mean scores of QOL were (73.2±13.8)scores. The dimension of “self-perception of appearance” was moderately positively correlated with the number of follow-up years, and the dimension of “pain” was moderately positively correlated with the Z-value of height(r=0.357, P=0.024; r=0.495, P=0.001). The dimension of “transplanting relationships with other people” was moderately negatively correlated with BMI and BMI Z value(r=-0.317, P=0.046; r=-0.384, P=0.032), and the dimension of “worry” was moderately negatively correlated with the Z-value of body weight(r=-0.319, P=0.045). Conclusion　The QOL of the pediatric patients who have received liver transplantation for 5 years is normal. The years of follow-up, BMI, height Z value, BMI Z value and weight Z value may be the factors affecting the long-term QOL after liver transplantation.

https://www.zglcxyxzz.com/zglcxyyen/ch/reader/view_abstract.aspx?file_no=20191005&flag=1

［Abstract］　Liver transplantation is the only effective treatment for acute liver failure which has a high mortality. However, due to the shortage of liver source, there is an urgent need for “bridge therapy” during the waiting list or liver self-regeneration. As a type of bioartificial liver, extracorporeal liver perfusion(ECLP) is expected to temporarily replace the failing livers in vivo and prolong the patients′ life. Since the 1950s, scholars have been studying and improving the technique of ECLP. With the development of new materials, new technologies and gene editing engineering, there has been a new upsurge of ECLP research in recent years. This article reviews the history, current situation and problems of ECLP, and discusses the future research direction in this field.
