<description xmlns:cf="http://www.microsoft.com/schemas/rss/core/2005" cf:type="html"><![CDATA[<b>［Abstract］</b>　This paper briefly interprets <i>the Guideline for the Prevention and Treatment of Diabetic Foot in China</i> issued in 2019 and <i>the International Clinical Guideline for the Prevention and Management of Diabetic Foot</i> issued by the International Working Group on Diabetic Foot(IWGDF). Emphasis should be placed on prevention, combination of prevention and treatment, hierarchical management and multidisciplinary team cooperation. The key factors of foot ulcer prevention are identifying the foot with ulcer risk, regularly assessing and inspecting the foot with ulcer risk, carrying out health education for the patients and their families and medical staff, ensuring proper shoes and managing the risk factors of foot ulcer. The complicated diabetic foot needs to be managed by a multidisciplinary team of diabetic foot disease.]]></description>

<description xmlns:cf="http://www.microsoft.com/schemas/rss/core/2005" cf:type="html"><![CDATA[<b>［Abstract］</b>　Diabetic foot is one of the serious chronic complications leading to disability and death of diabetic patients in China, which not only brings heavy psychological burden to the patients, but also causes heavy family and social burdens. Negative pressure wound therapy(NPWT) is used in recent years for the treatment of various acute and chronic wounds, and has achieved good results in the treatment of diabetic foot ulcers. NPWT includes two modes: vacuum assisted closure(VAC) and vacuum sealing drainage(VSD). This paper summarizes the development of NPWT and the mechanism of promoting wound healing, and compares the principles and functions of VAC and VSD.]]></description>

<title xmlns:cf="http://www.microsoft.com/schemas/rss/core/2005" cf:type="text"><![CDATA[Antibiotic resistance and virulence gene analysis of Pseudomonas aeruginosa in diabetic foot wound infection]]></title>

<description xmlns:cf="http://www.microsoft.com/schemas/rss/core/2005" cf:type="html"><![CDATA[<b>［Abstract］　Objective</b>　To investigate the trait of antibiotic resistance of Pseudomonas aeruginosa in diabetic foot wound infection and analyze the relationship between the antibiotic resistance and its virulence genes. <b>Methods</b>　Fifty-four strains of Pseudomonas aeruginosa were continuously collected from the diabetic foot patients in the Department of Diabetic Foot of Metabolic Disease Hospital of Tianjin Medical University during January 1, 2018 and December 31, 2018. In the bacterial culture report, the 54 strains were Pseudomonas aeruginosa. After the antibiotic sensitivity test was carried out,and the colonies were replanted and cultured, the inherent pcrV gene of Pseudomonas aeruginosa was detected one by one in the Microbial Laboratory of Nankai University, in order to identify the Pseudomonas aeruginosa. At the same time, 21 strains of Pseudomonas aeruginosa from nondiabetic wound infection were collected as the control group. The virulence genes exoS and exoU in the type 3 secretion system of Pseudomonas aeruginosa were detected and analyzed. The relationship between the antibiotic resistance and its virulence genes was analyzed. The Pseudomonas aeruginosa cultured in the wound for two times(one month interval) or more were followed up. <b>Results</b>　Eight out of fifty-four strains were not Pseudomonas aeruginosa, and the forty-six strains were Pseudomonas aeruginosa. In the diabetic foot group and the control group, the main pathogenic gene of Pseudomonas aeruginosa was exoS, with 91.3%(42/46) in the diabetic foot group in which the antibiotic resistance rate was 11.9%, and 76.2%(16/21) in the control group in which the antibiotic resistance rate was 6.3%. Pseudomonas aeruginosa with exoU was less in the diabetic foot group and the control group, with 8.7%(4/46) in the diabetic foot group in which the antibiotic resistance rate was 25.0%, and 23.8%(5/21) in the control group in which the antibiotic resistance rate was 20.0%. In the diabetic foot group, Pseudomonas aeruginosa was cultured twice or more in 6 patients with diabetic foot, with a total of 19 strains. Five patients were infected with Pseudomonas aeruginosa with exoS among whom 2 patients were cultured with drug-resistant Pseudomonas aeruginosa and the antibiotic resistance rate was 23.5%(4/17) and these conditions occurred in the early and middle stages of treatment, and they all turned into sensitive Pseudomonas aeruginosa in the last culture. In one patient, Pseudomonas aeruginosa with exoU was found in the wound, and the drug resistance rate was 50.0%(1/2). In this patient, the first culture was drug-resistant, and the last culture was sensitive. <b>Conclusion</b>　Among the strains of Pseudomonas aeruginosa isolated from diabetic foot wounds, the Pseudomonas aeruginosa with exoS is the main one. However, the antibiotic resistance rate of Pseudomonas aeruginosa with exoU is higher. The Pseudomonas aeruginosa in the same wound of a diabetic patient will not change its exoS or exoU gene because of changing of antibiotic resistance.]]></description>

<description xmlns:cf="http://www.microsoft.com/schemas/rss/core/2005" cf:type="html"><![CDATA[<b>［Abstract］　Objective</b>　To investigate the cognition degree and behavior status of the correlation between blood glucose control and diabetic foot in the outpatients with type 2 diabetes. <b>Methods</b>　Two hundred and eleven patients with type 2 diabetes were included in this study from the Outpatient Department of Endocrinology and Metabolism of West China Hospital of Sichuan University during October 1, 2017 and February 28, 2018. The general clinical data and glycosylated hemoglobin(HbA1c) were recorded. Questionnaire was used to collect the diabetic patients′ awareness and behavior to diabetic foot. <b>Results</b>　Among the 211 patients with type 2 diabetes, 125 cases(59.2%) believed that blood glucose control was related to the development of diabetic foot; 67 cases(31.8%) did not know that poor blood glucose control might lead to diabetic foot ulcers; 19 cases(9.0%) thought that the quality of blood glucose control had nothing to do with the occurrence of diabetic foot ulcers; 134 cases(63.5%) never had self examination of their feet. The diabetic patients′ awareness of the relationship between blood glucose control and diabetic foot was positively correlated with age and education. <b>Conclusion</b>　The awareness of diabetic patients to diabetic foot is not enough, especially the elderly diabetic patients with lower education level. Therefore, it is necessary to strengthen the education of early screening and prevention of diabetic foot so as to establish the idea that prevention is more important than treatment.]]></description>
