<description xmlns:cf="http://www.microsoft.com/schemas/rss/core/2005" cf:type="html"><![CDATA[<b>［Abstract］</b>　Venoarterial extracorporeal membrane oxygenation(VA-ECMO) is a quickly initiated and highly valuable heart-lung supporting treatment based on its effective blood transferring from venous to arterial vessels. VA-ECMO is more and more used in the treatment of severe acute cardiac dysfunction caused by various reasons after cardiac surgery.  As a highly effective extracorporeal life support therapy, the initiation of VA-ECMO, anticoagulation, weaning of VA-ECMO and other patient managements are still tough problems in clinical treatment. The particularities of patients after cardiac surgery make the management of VA-ECMO more difficult for such patients. It is necessary to systematically understand and master the norms of VA-ECMO in order to improve the survival rate of such patients.]]></description>

<description xmlns:cf="http://www.microsoft.com/schemas/rss/core/2005" cf:type="html"><![CDATA[<b>［Abstract］　Objective</b>　To explore the clinical application of extracorporeal membrane oxygenation(ECMO) in acute fulminant myocarditis(AFM) and its clinical effect. <b>Methods</b>　Ten AFM patients treated with ECMO were selected from the People′s Hospital of Guangxi Zhuang Autonomous Region during June 2019 and December 2020 to observe the therapeutic effect of ECMO. <b>Results</b>　The average time of ECMO support in the 10 AFM patients was (87.70±42.81)hours. Of the 10 patients, 7 patients were weaned from the ECMO smoothly and discharged from the hospital successfully. After ECMO treatment, the levels of brain natriuretic peptide(BNP), creatine kinase isoenzyme-MB(CK-MB), troponin I and lactic acid in the patients were significantly lower than those before treatment, and the differences were statistically significant(<i>P</i><0.05). Compared with that before treatment, the level of creatinine had no significant change after treatment(<i>P</i>>0.05). <b>Conclusion</b>　ECMO is an effective treatment for AFM, and can reduce the patients′mortality and improve their prognoses.]]></description>

<description xmlns:cf="http://www.microsoft.com/schemas/rss/core/2005" cf:type="html"><![CDATA[<b>［Abstract］　Objective</b>　To analyze the clinical characteristics of fulminant myocarditis in children and the therapeutic effect of extracorporeal membrane oxygenation(ECMO). <b>Methods</b>　A retrospective analysis was conducted on the clinical characteristics of fulminant myocarditis in 8 children treated in the People′s Hospital of Guangxi Zhuang Autonomous Region from July 2018 to December 2020 and the therapeutic effect of ECMO. <b>Results</b>　Of the 8 children with fulminant myocarditis, 5 received cardiopulmonary resuscitation and 4 developed multiple organ dysfunction syndrome. The ECMO support time was 91 hours(20-190 hours), and 7 cases were successfully weaned from ECMO, and 6 cases survived to hospital discharge. The neurological function of the surviving children was normal and their cardiac function recovered well. Complications occurred in 3 cases. Among the children with complications, 1 case had severe neurological complications and eventually died of brain death. <b>Conclusion</b>　Children with fulminant myocarditis often seek medical treatment because of extracardial symptoms, and are prone to malignant arrhythmia. ECMO is effective in treatment of fulminant myocarditis in children.]]></description>

<description xmlns:cf="http://www.microsoft.com/schemas/rss/core/2005" cf:type="html"><![CDATA[<b>［Abstract］　Objective</b>　To summarize the clinical experience of extracorporeal cardiopulmonary resuscitation(ECPR) as circulatory assist in adult patients with cardiac arrest. <b>Methods</b>　A total of 26 cardiac arrest patients who were admitted to the Emergency Intensive Care Unit(EICU) of the People′s Hospital of Guigang for ECPR treatment from April 2019 to December 2020 were collected. A retrospective analysis was performed on the patients′ medical records, and the clinical data, laboratory examination results, and various parameters and indicators were compared between the survival group and the death group. <b>Results</b>　Among the 26 patients, 9 cases were successfully weaned from extracorporeal membrane oxygenation(ECMO) and 6 cases survived to hospital discharge. All the survivors were the patients with in-hospital cardiac arrest(IHCA).Twenty cases died. The initial lactate value and the lactate values 6 and 12 hours after ECPR in the survival group were lower than those in the death group. The values of mean arterial pressure 6 and 12 hours after ECPR in the survival group were higher than those in the death group. The velocity time integral of the left ventricular outflow tract 6 and 12 hours after ECPR in the survival group was higher than that in the death group. There were significantly differences in the above indicators between the two groups(<i>P</i><0.05). <b>Conclusion</b>　ECMO can save the lives of some patients who have failed conventional cardiopulmonary resuscitation(CCPR) by providing emergency cardiopulmonary bypass assistance.]]></description>

However, the increase of left ventricular afterload is inevitable in VA-ECMO, and may lead to many complications including left ventricular dilation, left ventricular thrombosis, myocardial ischemia aggravation and pulmonary edema. This paper mainly reviews the current clinical reports and the use of left ventricular decompression strategies, aiming at early prevention and intervention of left ventricular dilation in the treatment of VA-ECMO.]]></description>
