阿尔茨海默病血脂学说新视角:与脑结构和功能相关性的MRI研究进展

摘要:
阿尔兹海默病(AD)是痴呆症最常见类型,其病理机制尚未完全阐明。血脂异常是多种慢性疾病的重要影响因素,也是AD的可控风险因素。国内外研究表明,血脂异常可能通过β-淀粉样蛋白及tau蛋白沉积、神经炎症反应、血脑屏障损伤与遗传易感性等多个方面,引起AD患者大脑的灰白质及脑网络连接等发生改变,进而导致认知障碍,但具体作用机制尚不明确。本综述分析了血脂异常与AD的潜在关联机制;讨论并总结了血脂异常引起阿尔兹海默病脑改变的MRI特点,从特定脑区灰质体积缩小、白质微结构损伤、脑网络连接减低等方面揭示了其相关性,为今后预防和治疗AD提供方向。

New perspective on Alzheimer's disease lipid theory: MRI research progress related to brain structure and function

Abstract:
Alzheimer"s disease (AD) is the most common type of dementia, and its pathological mechanism has not been fully elucidated. Dyslipidemia is an important influencing factor for various chronic diseases and a controllable risk factor for AD. Studies both domestically and internationally have shown that dyslipidemia may cause changes in the gray and white matter of the brain and brain network connectivity in AD patients through multiple aspects such as β-amyloid and tau protein deposition, neuroinflammatory response, blood-brain barrier damage, and genetic susceptibility, ultimately leading to cognitive impairment. However, the specific mechanism of action remains unclear. This review analyzes the potential mechanisms of association between dyslipidemia and AD; discusses and summarizes the MRI characteristics of brain changes caused by dyslipidemia in Alzheimer"s disease, revealing its correlation from aspects such as reduced gray matter volume in specific brain regions, white matter microstructural damage, and decreased brain network connectivity, providing directions for the prevention and treatment of AD in the future.
