糖尿病肾病肾纤维化最新研究进展

摘要:
糖尿病肾病是糖尿病最主要的微血管并发症之一,肾纤维化是其进展至终末期肾脏病的核心病理环节。该文系统综述糖尿病肾病肾纤维化的主要发病机制,包括氧化应激、炎症反应、关键细胞因子及相关信号通路的作用,并总结以TGF-β/Smad、PI3K/Akt等为代表的靶向治疗进展。同时,对维生素D受体、NLRP3炎症小体等新兴靶点及中药多成分调控策略进行探讨,以期为临床抗纤维化治疗提供理论依据,并拓展糖尿病肾病的未来治疗思路。

Abstract:
Diabetic nephropathy (DN) is a major microvascular complication of diabetes. Renal fibrosis is the core pathological feature driving its progression to end-stage renal disease. This systematic review summarizes the primary pathogenic mechanisms of diabetic nephropathy and renal fibrosis, including the roles of oxidative stress, inflammatory responses, key cytokines, and related signaling pathways (such as TGF-β/Smad, PI3K/Akt, Wnt/β-catenin, etc.). In the realm of therapeutic progress, this paper offers a comprehensive overview of conventional targeted intervention strategies and an in-depth exploration of the mechanisms of emerging therapeutic targets, such as vitamin D receptors and the NLRP3 inflammasome. It systematically analyzes the distinctive advantages of multi-component, multi-target regulation strategies in traditional Chinese medicine. This review aims to provide a theoretical foundation for clinical anti-fibrosis treatment and to expand the future treatment options for DN.
