维持性血液透析患者支持性照顾需求的现状及影响因素分析

摘要:
[摘要]目的 分析维持性血液透析(MHD)患者支持性照顾需求的现状及影响因素,为临床早期识别高需求人群、制定个体化干预措施提供依据。方法 采取便利抽样法,选取武汉科技大学附属武昌医院2025年3至7月200例门诊血液透析患者作为研究对象。采用一般资料调查问卷、支持性照顾需求量表(SCNS-SF34)、终末期肾病维持性血液透析患者治疗依从性量表、汉密尔顿焦虑量表(HAMA)、汉密尔顿抑郁量表(HAMD)对患者进行问卷调查。采用Pearson相关分析MHD患者支持性照顾需求、焦虑抑郁和治疗依从性之间的相关性。采用多重线性回归分析MHD患者支持性照顾需求影响因素。结果 不同年龄、教育水平、工作情况、平均月收入的MHD患者SCNS-SF34评分比较差异有统计学意义(P<0.05)。200例MHD患者SCNS-SF34总分为(78.98±14.86)分。MHD患者支持性照顾需求与HAMA评分呈正相关(r=0.774,P<0.001)、与HAMD评分呈正相关(r=0.322,P<0.001);治疗依从性与HAMA评分呈负相关(r=-0.346,P<0.001)、与HAMD评分呈负相关(r=-0.224,P=0.001);支持性照顾需求与治疗依从性呈负相关(r=-0.466,P<0.001)。多重线性回归分析结果显示,年龄、教育水平、工作情况、焦虑程度、治疗依从性是MHD患者支持性照顾需求的影响因素(P<0.05)。结论 MHD患者存在未满足的支持性照顾需求,且受年龄、教育水平、工作情况、HAMD评分、治疗依从性的影响。

Current Status and Influencing Factors of Supportive Care Needs Among Patients Receiving Maintenance Hemodialysis

Abstract:
[Abstract] Objective To investigate the current status of supportive care needs and their influencing factors among patients undergoing maintenance hemodialysis (MHD), and to provide evidence for the early identification of patients with high supportive care needs and the development of individualized clinical interventions. Methods Using a convenience sampling method, outpatient hemodialysis patients undergoing MHD at Wuchang Hospital Affiliated to Wuhan University of Science and Technology from March to July 2025 were recruited as the study participants. Data were collected using a general information questionnaire, the Supportive Care Needs Survey–Short Form 34 (SCNS-SF34), the End-Stage Renal Disease Maintenance Hemodialysis Patient Treatment Adherence Scale, the Hamilton Anxiety Scale (HAMA), and the Hamilton Depression Scale (HAMD). Pearson correlation analysis was used to examine the correlations among supportive care needs, anxiety and depression, and treatment adherence in patients undergoing MHD. Multiple linear regression analysis was performed to identify the factors influencing supportive care needs in patients with MHD. Results There were statistically significant differences in SCNS-SF34 scores among patients undergoing MHD with different ages, educational levels, employment status, and average monthly income (P < 0.05). The total score of SCNS-SF34 among the 200 patients undergoing MHD was (78.98±14.86). Supportive care needs in patients undergoing MHD were positively correlated with HAMA scores (r = 0.774, P < 0.001) and HAMD scores (r = 0.322, P < 0.001). Treatment adherence was negatively correlated with HAMA scores (r = -0.346, P < 0.001) and HAMD scores (r = -0.224, P = 0.001). In addition, supportive care needs were negatively correlated with treatment adherence (r = -0.466, P < 0.001). Multivariate analysis showed that age, education level, work or study status, anxiety level, and treatment adherence were significant influencing factors of supportive care needs in patients undergoing MHD (P < 0.05). Conclusion Patients undergoing MHD have unmet supportive care needs, which are influenced by age, education level, work or study status, anxiety level, and treatment adherence. These findings provide evidence for clinical healthcare professionals to accurately identify differences in supportive care needs among MHD patients and may help further optimize the care service system for this population.
