卵巢组织冻存与移植技术在白血病患者中的临床应用进展

摘要:
卵巢组织冻存移植技术的有效性与安全性得到广泛验证,已成为临床常规技术。对于拟行造血干细胞移植(HSCT)的白血病患者,HSCT前的预处理方案可导致卵巢功能严重且不可逆的损害,进而影响其生育潜力及卵巢内分泌功能。因此,在该人群中进行卵巢组织冻存与移植具有重要的临床意义。然而,由于存在潜在的白血病细胞通过移植的卵巢皮质片回输的风险,其安全性仍是制约该技术在此类人群中广泛实施的关键问题。本文系统综述卵巢组织冻存在白血病患者中的应用、白血病细胞经卵巢皮质移植回输的风险评估、冻存卵巢组织在白血病幸存者中的移植结局,以期为临床决策与实践提供有价值的参考和指导。

Abstract:
Ovarian tissue cryopreservation and transplantation (OTCT) has been widely validated for its efficacy and safety, and has become a routine clinical technique. For leukemia patients scheduled to undergo hematopoietic stem cell transplantation (HSCT), the preconditioning regimen before HSCT can cause severe and irreversible damage to ovarian function, thereby affecting their fertility potential and ovarian endocrine function. Therefore, OTCT holds significant clinical importance for this population. However, the potential risk of reintroducing leukemia cells through the transplanted ovarian cortical fragments remains a key safety concern limiting the widespread application of this technique in such patients. This article systematically reviews the application of ovarian tissue cryopreservation in leukemia patients, assesses the risk of leukemia cell transmission through ovarian tissue transplantation, and evaluates the outcomes of cryopreserved ovarian tissue transplantation in leukemia survivors, aiming to provide valuable references and guidance for clinical decision-making and practice.
