女性生育力保存的技术选择策略与适应证解读:现状、挑战与未来展望

摘要:
随着恶性肿瘤生存率的提高、社会婚育年龄的普遍推迟以及免疫/ 代谢/遗传性疾病诊疗的进展,女性生育力保存(female fertility preservation, FP)已发展为生殖医学与肿瘤学、血液学、免疫学、代谢学、临床遗传学等多学科交叉的重要临床领域。目前,已形成以胚胎冷冻、卵母细胞冷冻和卵巢组织冷冻与移植(ovarian tissue cryopreservation and transplantation, OTCP)为核心的女性生育力保存技术体系。本文基于国内外权威指南与循证医学证据,系统梳理女性FP技术的临床应用现状与优劣特征,深入解读从疾病相关指征到社会性需求的适应证谱系,构建 “人群分层 - 需求导向 - 技术适配” 的个体化选择策略 [3]。同时,剖析当前面临的技术争议、临床可及性不足、多学科协作(multidisciplinary team, MDT)机制不完善及伦理法律困境等核心挑战,并对未来技术创新、体系优化与政策支持方向进行展望,旨在为临床规范化与精准化决策提供参考,推动我国女性生育力保存事业的健康发展。

Strategies for Female Fertility Preservation: Interpretation of Technical Options and Indications – Current Status, Challenges, and Future Perspectives

Abstract:
With the increasing survival rates of malignant tumors, the overall trend toward delayed marriage and childbearing, and advances in the diagnosis and treatment of immune, metabolic, and genetic diseases, female fertility preservation (FP) has emerged as a pivotal interdisciplinary field spanning reproductive medicine, oncology, hematology, immunology, metabolomics, clinical genetics, and related specialties. Currently, a structured technical framework for female FP has been established, centered on embryo cryopreservation, oocyte cryopreservation, and ovarian tissue cryopreservation and transplantation (OTCP). Drawing on authoritative domestic and international guidelines and evidence-based medical research, this article systematically reviews the current clinical application status, advantages, and limitations of female FP technologies. It provides an in-depth interpretation of the indication spectrum—ranging from disease-related needs to social demand—with the aim of developing an individualized selection strategy characterized by population stratification, demand orientation, and technology adaptation [3]. Furthermore, the article examines core challenges currently faced in the field, including technical controversies, limited clinical accessibility, insufficient multidisciplinary team (MDT) coordination, and ethical and legal dilemmas. Finally, it outlines future directions in technological innovation, system optimization, and policy support. This review is intended to serve as a reference for standardized and precise clinical decision-making and to contribute to the healthy development of female fertility preservation in China.
