摘要:
目的 构建老年人群饮食结构与认知功能障碍之间的临床预测模型。方法 采用中国纵向健康长寿调查(CLHLS)2018年的数据,该调查对象为60岁及以上的社区居民。研究人员进行了全面的健康评估,涵盖影响整体健康的多项指标。采用简易精神状态检查量表(MMSE)对认知功能进行详细评估,并运用多因素Logistic回归模型分析老年人认知障碍独立影响因素,并据此构建预测老年人认知障碍的列线图。结果 多因素logistic回归模型筛选出12个老年人认知障碍的独立影响因素,据此构建出包含12个预测因子的列线图,包括年龄、性别、主食类型、蔬菜、鱼类、蛋类、奶制品、坚果、海藻、维生素补剂、饮酒、体重指数等。其区分度良好,训练集的曲线下面积(area under the curve of the prediction model, AUC)为0.787。结论 该列线图经验证在预测老年人认知功能障碍方面具有一定的临床价值,所包含的临床预测因子均可从日常生活评估中便捷获取。

To Develop a nomogram for predicting dietary structure and risk of cognitive impairment in older adults

Abstract:
Objective To develop a clinical predictive model for cognitive impairment combined with dietary structure in older adults. Methods This study utilized data from the 2018 China Longitudinal Health and Longevity Survey(CLHLS), which included community residents aged 60 and older. Researchers conducted a comprehensive health assessment covering multiple indicators affecting overall health. Cognitive function was evaluated in detail using the Mini-Mental State Examination (MMSE), and a multivariate logistic regression model was employed to analyze independent risk factors for cognitive impairment in older adults. Based on these findings, a nomogram was developed to predict cognitive impairment in older adults. Results A multivariate logistic regression model identified 12 independent predictors of cognitive impairment in older adults. Based on these findings, a nomogram incorporating these 12 predictors was developed, encompassing age, sex, staple food, vegetable consumption, fish consumption, egg consumption, dairy consumption, nut consumption, seaweed consumption, vitamin supplementation, alcohol consumption, and body mass index (BMI). The nomogram demonstrated good discriminative ability, with an area under the curve (AUC) of 0.787 for the training set.Conclusion This nomogram has been validated to have clinical value in predicting cognitive impairment in older adults. All clinical predictors included can be easily obtained through assessments of daily living.
