| ［摘要］ 目的 比较和评价硝普钠、多巴胺、多巴酚丁胺三联微泵注射治疗顽固性心衰(ＲＨＦ)的疗效。方法 常规给予吸氧、镇静、制动、节食、抗感染，强心、利尿及扩血管治疗。治疗组给予硝普钠50 mg溶于5%葡萄糖溶液50 ml中, 避光, 以1 ml/h起持续微泵注入, 根据血压调节速度, 最快不超过4 ml/h, 24 h更换。多巴胺200 mg+多巴酚丁胺100 mg溶于5%葡萄糖溶液50 ml中, 以2 ml/h起持续微泵注入, 根据血压调节速度，最快不超过8 ml/h, 24 h更换。对照组继续给予洋地黄、利尿剂、硝酸甘油等常规治疗, 两组其他治疗相同, 7 d为一疗程。结果 治疗组总有效率93.7%, 副反应率15.3%, 对照组总有效率75.0%, 副反应率15.6%, 总有效率两组对比差异有统计学意义(P均<0.01)。副反应率无差异。结论 硝普钠、多巴胺、多巴酚丁胺三联微泵注射治疗ＲＨＦ合理、安全、疗效显著。 |

| ［Abstract］ Objective To evaluate the therapeutic effects of sodium nitroprusside, dopamine and dobutamine on patients with refractory heart failure by continuing minipumpinjections.Methods All patients were gived oxygen, appeasement, reducing activity and food, anti-infection.The treatment group were gived with sodium nitroprusside by a speed of 1-4mg/hour, dopamine, 8-32mg/hour and dobutamine 4-16mg/hour.The control group were given digitalis. The course of treatment was seven days.Results The total effective rate of the treatment group was 93.7% and the control group was 75.0%.The contrast difference was significance (P<0.01). Conclusion The therapeutic effects of sodium nitroprusside, dopamine and dobutamine on patients with refractory heart failure are significant. |
