窒息新生儿生后72 h内电解质变化的临床观察

摘要:
[摘要] 目的 回顾分析窒息新生儿生后0~72 h电解质水平变化及电解质紊乱情况。方法 选择符合观察条件的窒息新生儿23例,非窒息新生儿17例,分0~24 h、25~48 h、49~72 h三个时间段抽取静脉血2 ml检查,对照两组血钠、钾、氯、钙、磷、镁等电解质水平变化情况及低钠、钾、钙、镁血症发生率的改变。结果 生后三个时间段血钠、血钙质量水平窒息组明显低于非窒息组,对比差异有统计学意义(P<0.05);血镁水平在25~48 h和49~72 h时间段窒息组明显低于非窒息组,对比差异有统计学意义(P<0.05),而在0~24 h这个时间段中对比差异无统计学意义(P>0.05);血钾质量水平仅在49~72 h时间段窒息组明显低于非窒息组,对比差异有统计学意义(P<0.05);氯、磷水平在各个时间段窒息组和非窒息组无明显变化,对比差异无统计学意义(P>0.05);窒息新生儿在生后各时间段都可发生低血钠、低血钾、低血钙,以25~48 h时间段为甚,而低血镁则多发生在25~48 h、49~72 h两个时间段。结论 窒息新生儿生后的72 h内易发生电解质紊乱,应在不同时间段视血气、电解质水平变化情况采取相应措施予以纠正电解质紊乱。

Abstract:
[Abstract] Objective To analyse the changes of electrolytes in neonates with asphyxia within 72 hours after birth retrospectively.Methods Twenty-three neonates suffered from asphyxia were chosen as the asphyxia group and 17 neonates without asphyxia were chosen as the non-asphyxia group. Venous blood samples of 2 ml were drawn at 0~24 hours, 25~48 hours and 49~72 hours respectively. Then the data of the two groups were compared on the changes of electrolytes (such as plasma sodium, potassium, chlorine, calcium and magnesium) and the incidence of hyponatremia, hypokalemia, hypocalcaemia and hopomagenesemia.Results The rates of hyponatremia and hypocalcaemia in asphyxia group were much higher than those in non-asphyxia group in all of the 3 periods. There were significant difference between the two groups(P<0.05).The level of plasma magnesium was much higher in asphyxia group in periods of 25~48 hours and 49~72 hours. There were significant differences in 25~48 hours and 49~72 hours(P<0.05) while no significant difference in 0~24 hours(P>0.05). Only in the period of 49~72 hours, the level of plasma potassium in asphyxia group was significantly lower than that in non-asphyxia group (P<0.05). However, there were no differences in plasma chlorine and phosphonium between the two groups(P>0.05).Conclusion Electrolytes disorders can appear not only in 24 hours but also in different periods after asphyxia, therefore, it is necessary to maintain homeostasisb by taking corresponding measures according to the changes of blood gas and the changes of electrolytes.
