[摘要] 目的 探讨原发性非小细胞肺癌(NSCLC)患者手术前后外周血中T细胞亚群的定量变化及其临床意义。方法 将原发性非小细胞肺癌Ⅱ~Ⅲb进行手术治疗的患者40例作为研究组(A组),肺部良性肿瘤进行手术的患者15例作为对照组(B组),40例正常健康成人作为基础对照组(C组),于术前取全组外周静脉血检测T细胞亚群和NK细胞活性;术后第10天取A组和B组外周血再检测T细胞亚群和NK细胞活性。结果 术前A组T细胞亚群和NK细胞与B组和C组的比较,除CD8+外均明显降低(P<0.05)。术后完全性切除和不完全性切除的患者CD8+低于剖胸探查者(P<0.05),其余各项指标均高于剖胸探查组(P<0.05),而完全性切除与不完全性切除之间差异无统计学意义(P>0.05)。结论 检测T淋巴细胞亚群、NK细胞可用于肺癌患者的免疫监测,其中CD4+/CD8+的比值在一定程度上可以反映原发性非小细胞肺癌患者癌负荷的情况。完全性切除和不完全性切除都能显著性提高肺癌患者的免疫功能,为放疗或化疗取得更好的疗效创造较好的条件。 |

[Abstract] Objective To study the clinical significance of changes of the level of T lymphocyte subsets and NK cells in peripherial blood of early metaphase patients with NSCLC.Methods Flow cytometry was employed to detect T lymphocyte subsets and NK cells in peripherial blood from 40 patients with NSCLC(Ⅱ~Ⅲb ) (A group) and 15 patients with benign tumor of lung (B group) needed surgery with 40 healthy adults as the controls(C group).The blood specimens were taken on the postoperative 10th days and T lymphocyte subsets and NK cells were defected in A group and B group.Results Compared with the B group and the C group, the Indicators were significantly higher (P<0.05) in the A group except CD8+. The level of CD8+ cells were significantly higher (P<0.05) in the group of thoracic exploration than the group of complete resection and partial resection on the postoperative 10th days. The significant differences between the group of complete resection and partial resection did not be found(P>0.05).Conclusion Monitoring T lymphocyte subsets and NK cells in peripheral blood can be used to detect the patients immunologic function of patients with NSCLC. Especially The CD4+/CD8+ ratio can reflect the pathogenetic condition of patients with NSCLC. Complete resection and partial resection can improve the immunologic function of patients with NSCLC. |
