超声检测脐动脉、脐静脉内径在地中海贫血及正常胎儿的对比研究

摘要:
[摘要] 目的 探讨超声检测胎儿脐动脉、脐静脉内径及动静脉内径比值在地中海贫血(地贫)胎儿产前筛查的价值。方法 对120例经羊膜腔穿刺或脐血穿刺术进行地贫基因诊断为地贫的孕12~40周单胎孕妇(地贫组)行二维及多普勒超声检查,测量胎儿脐动脉、脐静脉内径,计算动静脉内径比值,选择同期正常胎儿106例作为对照组,比较两组间上述指标。结果 地贫组中孕期胎儿动脉、静脉内径均明显高于对照组,动、静脉内径比值低于对照组,差异有统计学意义(P<0.05);晚孕期上述指标差异无明显统计学意义(P>0.05)。结论 超声测量中孕期胎儿脐动脉、脐静脉内径以及动静脉内径比值作为一种非侵人性方法对预测胎儿地贫有一定的价值。

Abstract:
[Abstract] Objective To evaluate the value of the diameter of fetal umbilical artery and umbilical vein in the prenatal diagnosis of thalassemia.Methods One hundred and twenty fetus of thalassemia verified by molecular biologic technique were detected by two-dimensional echocardiography and Doppler ultrasonography;106 normal fetus were used as control group. The diameters of fetal umbilical artery and umbilical vein were detected by ultrasound, the diameter ratio(umbilical artery / umbilical vein) was measured. Results The diameters of fetal umbilical artery and umbilical vein of thalassemia group were obviously larger than those of control group, there was statistical significance (P<0.05). The diameter ratio of thalassemia group was bviously lower than that of control group, there was statistical significance (P<0.05). Conclusion As a non-invasive technique, ultrasound detection of the diameter of fetal umbilical artery and umbilical vein has a certain value in the prenatal diagnosis of thalassemia.
