| ［摘要］ 目的 探讨成人股骨头缺血性坏死行人工关节置换术后的疗效。方法 回顾性分析72例成人股骨头缺血性坏死患者行人工关节置换术后的临床资料。72例中男59例，女13例；年龄38～75岁，平均62岁，其中38～60岁18例，61～70岁47例，71～75岁7例。酒精性股骨头缺血性坏死32例，股骨颈骨折、髋关节脱位27例，皮质类固醇应用性股骨头缺血性坏死10例，原因不明3例。双侧股骨头坏死7例，单侧65例。全组合并有心血管疾病者38例，慢性支气管炎者6例。单纯人工股骨头置换术20例，人工全髋置换术52例，其中骨水泥型26例，非骨水泥型23例，混合型3例。二期人工关节翻修术4例。结果 72例全部治愈。其中合并人工关节脱位3例，合并深静脉血栓形成3例，合并人工假体股骨柄断裂1例，合并应激性溃疡1例。随访时间2年～10年6个月，平均5年6个月。Harris评分由术前的40.3分增加到术后的91.5分。结论 成人晚期股骨头缺血性坏死患者进行人工髋关节置换术疗效良好。 |

| ［Abstract］ Objective To observe the postoperative effects of artificial joint in adult patients with avascular necrosis of the femoral head.Methods Postoperative effects of artificial joint in 72 adult patients with avascular neorosis of the femoral head were retrospectiveiy analyzed.Fifty-nine cases were male and 13 female aged 38～75 years,and 36～60 years old in 18 cases,61～70 years old in 47 cases,71～75 years old in 7 cases,with an average age of 62 years.Of them alcoholic avascular necrosis of the femoral head were diagnosed in 32 cases,femoral neck fracture and dislocation of the hip in 27 cases; avscular necrosis of the femoral head due to corticoids in 10 cases,unknown cause in 3 cases;double side osteonecrosis of femoral head in 7 cases,the one-side in 65 cases; 38 cases suffered from cardiovascular diseases and 6 cases suffered from chronic bronchitis.The types of the operation included prosthetic replacement for femoral head in 20 cases,total hip arthroplasty in 52 cases.Twenty-six cases were treated by cement total hip arthroplasty, 23 cases were treated by cementless total hip arthroplasty, and 3 cases were treated bigmixed cement, of them artificial joint were rebuilt in 4 cases. Results The 72 cases were cured. Dislocation of artificial joint were found in 3 cases; deep vein thrombosis in 3 cases. Break off artificial femoral handle in 1 case;stress ulcer in 1 case. The follow-up period ranged from 2 years to 10 years and 6 months, with an average of 5.6 years. The mean Harris hip score increased from 40.3 points before operation to 91.5 points after operation. Conclusion Effects of artificial hip arthroplasty for avascular necrosis of the femoral head are exallent. |
