[摘要] 目的 探讨联合检测多种生化指标对高血压肾损害早期诊断的价值。方法 将入选的98例高血压患者分为A、B两组,A组为单纯高血压患者(n=50),B组为高血压合并有微量蛋白尿患者(n=48),同时选择56名健康体检者作为对照组,分别检测血清肌酐(Scr)、血清胱抑素C(Cys C)、尿β2微球蛋白(β2-MG)、尿N-乙酰-β-D-氨基葡萄糖苷酶(N-acetyl-β-D-glucosaminidase,NAG)四项指标。结果 三组Scr检测值比较差异无统计学意义(P>0.05);A组、B组Cys C、β2-MG、NAG水平均明显高于对照组(P<0.01),而B组Cys C、β2-MG、NAG水平明显高于A组(P<0.01)。由Cys C、β2-MG、NAG组成的生化标志组合对高血压肾损害早期的阳性诊断率达69.3%(68/98),明显高于Cys C 36.7%(36/98)、β2-MG 28.5%(28/98)和NAG 33.6%(33/98)各单项检测(P<0.01)。结论 多种生化指标联合检测可较早地反映高血压肾功能受损程度,对高血压肾损害早期的诊断具较高的敏感性和临床意义。 |

[Abstract] Objective To investigate the value of combined determination of a lot of biochemical index in early diagnosis of hypertensive renal damage. Methods Ninety-eight hypertension patients were divided into A group and B group. Hypertension patients without complication were used as group A (n=50), those with microalbuminuria were used as group B (n=48), and 56 healthy persons were used as control group. Scr, Cys-C, β2-MG, N-acetyl-β-D-glucosaminidase (NAG) were measured respectively.Results Scr had no significant difference among the three groups(P>0.05),and the levels of Cys-C, β2-MG and NAG in group A and B was higher than those in control group(P<0.01), and the levels of Cys-C, β2-MG and NAG in group B was higher than those in group A (P<0.01).The positive diagnosis rate of combined determination of Cys-C、β2-MG and NAG in early diagnosis of hypertensive renal damage can reach 69.3%(68/98), which was higher than Cys-C 36.7%(36/98), β2-MG 28.5%(28/98) and NAG 33.6%(33/98) which was detected by single index(P<0.01).Conclusion Combined determination of blood and urine can reflect hypertensive renal functional lesion early, and have high sensitivity and clinical significance in early diagnosis of hypertensive renal damage. |
