社区获得性肺炎患儿Clara细胞分泌蛋白的变化及临床意义

摘要:
[摘要] 目的 探讨Clara细胞分泌蛋白(CCSP)在儿童社区获得性肺炎中的临床意义。方法 采用酶联免疫吸附法检测34例社区获得性肺炎患儿(肺炎组)血清及痰液Clara细胞分泌蛋白水平,治疗10~14 d临床症状、体征好转后复查其血清CCSP浓度。另设22名健康儿童作为对照组。结果 与健康对照组比较,社区获得性肺炎患儿血清CCSP浓度明显降低,治疗好转后其血清CCSP恢复至正常水平。无论是重症肺炎患儿还是轻症肺炎患儿,其血清CCSP浓度均低于健康对照组,其中轻症肺炎组血清CCSP水平较重症肺炎组下降更为显著,但差异无统计学意义;重症肺炎组痰液CCSP浓度较轻症肺炎组低,两组间差异有统计学意义;轻症肺炎患儿血清与痰液CCSP浓度呈正相关,但重症肺炎血清与痰液CCSP浓度无相关性。结论 CCSP参与肺炎发生、发展过程,对病情判断有一定指导意义。

Variation and clinical significance of clara cell secretory protein in children with community acquired pneumonia

Abstract:
[Abstract] Objective To explore the clinical significance of Clara cell secretory protein(CCSP)in children with community acquired pneumonia(CAP). Methods Thirty-four children with CAP and twenty-two healthy controls were enrolled.Venous blood samples and sputum samples were collected in CAP children,and blood samples alone were obtained in control subjects.After 10~14 days of treatment,blood samples were obtained in children with CAP without clinical symptoms and signs. The levels of CCSP were measured in sera and sputum by enzyme-linked immunosorhent assay(ELISA).Results Children with CAP,compared with controls,had significantly lower concentration of CCSP in sera(P<0.01).After recovery from pneumonia,the levels of CCSP in children with CAP increased to normal levels(P>0.05); In both severe CAP group and mild CAP group,the levels of sera CCSP in CAP children were significantly lower than those in control group,especially those in mild CAP group, but there were no difference between two CAP groups(P>0.05); In severe CAP group,the levels of CCSP in sputum were significantly lower than those in mild CAP group(P<0.01); In mild CAP group,there were significant positive correlation of CCSP levels between sera and sputum (P<0.01).Unfortunately,in severe CAP group,there were no correlation of CCSP concentration between sera and sputum(P>0.05).Conclusion CCSP may play a protective role in the pathogenesis and development of CAP.And the CCSP levels may preferably evaluate the state of children with CAP.
