神经元特异性烯醇化酶对癌性胸水诊断价值的Meta分析

摘要:
[摘要] 目的 探讨测定胸水中神经元特异性烯醇化酶(NSE)水平对癌性胸水的诊断价值。方法 制定纳入标准对相关文献进行采集,然后通过随机效应模型汇总并提炼出Meta分析所需指标,计算出NSE诊断癌性胸水的灵敏度、特异度以及其他指标,绘制汇总受试者工作特征(ROC)曲线并分析其诊断价值。结果 最终有10项独立研究符合纳入标准被收入此次Meta分析,NSE诊断癌性胸水的总体灵敏度为0.29(95%CI 0.25~0.33),特异度为0.87(95%CI 0.84~0.89),阳性似然比为3.67(95%CI 2.06~6.54),阴性似然比为0.77(95%CI 0.63~0.95),诊断优势比为5.24(95%CI 2.22~12.38)。结论 NSE诊断癌性胸水的灵敏度、特异度不高,测定胸水中NSE水平对癌性胸水的诊断价值不大。分析胸水的NSE水平应该结合临床表现以及常规的检查结果。

Abstract:
[Abstract] Objective To evaluate the diagnostic value of neuron-specific enolase(NSE) in malignant pleural effusion. We selected relevant literatures to undertake a meta analysis for evaluating the diagnostic value of NSE in malignant pleural effusion.Methods The inclusion criteria to selected literatures was formulated. Required indicators were extracted from selected literatures and pooled by using random-effects models to determine sensitivity, specificity, and other measures of accuracy of NSE concentrations in the diagnosis of pleural effusion. Summary receiver operating characteristic curves were used to summarize overall test performance.Results Ten studies met our inclusion criteria and were selected eventually. The summary estimates for NSE in the diagnosis of malignant pleural effusion in the studies included were as follows: sensitivity, 0.29(95%CI 0.25~0.33); specificity, 0.87(95%CI 0.84~0.89); positive likelihood ratio, 3.67(95%CI 2.06~6.54); negative likelihood ratio,0.77(95%CI 0.63~0.95); and diagnostic odds ratio, 5.24(95%CI 2.22~12.38).Conclusion NSE determination is not a sensitive and specific test for the diagnosis of malignant pleural effusion. The measurement of NSE levels in pleural effusions is not a useful tool for differentiation between malignant and benign pleural effusion. NSE levels in pleural effusions should be interpreted in parallel with clinical manifestations and the results of conventional examinations.
