利培酮联合低频重复经颅刺激改善老年精神分裂症患者顽固性幻听和认知功能的研究

摘要:
[摘要] 目的 探讨低频重复经颅刺激对于改善老年精神分裂症患者顽固性幻听和认知功能的临床应用价值。方法 按就诊顺序编号将100例老年精神分裂症患者分为对照组和观察组,每组50例。对照组给予利培酮治疗;观察组联用利培酮与低频重复经颅刺激治疗。于治疗前后采用听幻觉量表(AHRS)及副反应量表(TESS)对患者进行测评,同时评估患者的词语流畅性及连线实验表现。结果 经8周治疗后,观察组的幻听评分明显低于对照组,组间比较差异具有统计学意义(P<0.05)。观察组词语流畅性明显优于治疗前(P<0.05)。对照组词语流畅性治疗前后比较差异无统计学意义(P>0.05)。连线实验结果显示,两组患者治疗前后比较差异均无统计学意义(P>0.05)。结论 低频重复经颅刺激可有效改善老年精神分裂症患者顽固性幻听,对改善其认知功能具有一定效果,值得临床借鉴和推广。

Abstract:
[Abstract] Objective To explroe the clinical value of low frequency repetitive transcranial magnetic stimulation in improving obstinate auditory hallucinations and cognitive function in patients with schizophrenia.Methods According to visit order number 100 patients with schizophrenia were divided into control group and observation group,with 50 cases in each group. The control group was treated with risperidone. The observation group was treated by risperidone combined with low frequency repetitive transcranial magnetic stimulation.Before and after treatment, the patients were evaluated, by auditory hallucinationsrating scale(AHRS) and Treatment Emergent Symptom Scale(TESS) and assessment of verbal fluency and trail making were performal in patients.Results After 8 weeks of treatment,the AHRS scores in the observation group were lower than that in the control group, with statistical significance between two groups(P<0.05); The observation group′s verbal fluency obviously improved than before treatment(P<0.05), the control group′s verbal fluency did not show no significant improvement(P>0.05).There was no significant difference in trail making between the two groups before and after treatment(P>0.05).Conclusion Low frequency repetitive transcranial magnetic stimulation can effectively improve obstinate auditory hallucinations in elderly patients with schizophrenia, and has certain effect in improving their cognitive function,and is worthy of clinical reference and promotion.
