| ［摘要］ 目的 探讨强心利尿剂治疗顽固性心力衰竭的疗效。方法 选择36例经常规强心、利尿、扩血管［洋地黄、磷酸二酯酶抑制剂、转换酶抑制剂（ACEI）、速尿］等治疗无效的顽固性心力衰竭患者应用多巴胺、多巴酚丁胺、硝酸甘油、氨茶碱、托拉塞米等小剂量针剂组成的强心利尿剂进行治疗。结果 显效（由心功能Ⅳ级减轻到～Ⅱ级）25例，有效（心功能Ⅳ级减轻到Ⅱ～Ⅲ级）7例，无效（心衰症状无明显改变）4例。治疗前后心率、血流动力学、左室射血分数（LVEF）、左室收缩末径（LVESD）等指标比较差异均有统计学意义（P<0.01）。结论 多巴胺、多巴酚丁胺、硝酸甘油、氨茶碱、托拉塞米组成的小剂量强心利尿剂治疗顽固性心力衰竭效果好。 |

| ［Abstract］ Objective To investigate the curative effect of cardiotonic and diuretic mixture in the treatment of refractory heart failure.Methods Thirty-six refractory heart failure patients who had not response to conventional cardiac, diuretic, vasodilator (digitalis, phosphodiesterase inhibitors, ACEI, furosemide and so on) were treated by cardiotonic and diuretic mixture composed of small dose of dopamine, dobutamine, nitroglycerin, aminophylline, torasemide injection etc.Results The marked effect was found in 25 cases(NYHA decreased from grade Ⅳ toⅠ～Ⅱ), effectively in 7 cases(NYHA decreased from grade Ⅳ to Ⅱ～Ⅲ), no effect in 4 cases(heart failure symptoms had no obvious change). There were statistically significant differences in heart rate, hemodynamic left ventricular ejection fraction(LVEF), left ventricular end-systolic diameter(LVESD) and other indicators before and affer treatment(P<0.01).Conclusion The cardiofonic diuretic mixture composed of dopamine, dobutamine, nitroglycerin, aminophylline, torasemide injection etc has marked effect in treating refractory heart failure patients who had not response to conventional cardiotonic and diuretic therapy. |
