腹膜透析相关腹膜炎常见致病菌分布及耐药性检验分析

摘要:
[摘要] 目的 探讨腹膜透析相关腹膜炎常见致病菌分布及其耐药性。方法 对2013-06~2014-10该院收治的60例腹膜透析相关腹膜炎患者的临床资料进行回顾性分析。结果 微生物培养结果阳性29例(48.33%),其中革兰阳性球菌12例,革兰阴性杆菌14例,真菌3例;革兰阴性杆菌对氨苄西林的耐药率最高,对亚胺培南的耐药率最低,分别为78.57%、16.67%。革兰阳性球菌对青霉素的耐药率最高,为76.92%,对环丙沙星的耐药率次之,为71.43%。结论 革兰阳性球菌与革兰阴性杆菌均是导致腹膜透析相关腹膜炎的常见致病菌;提高腹膜透析液细菌培养阳性率对抗感染治疗具有重要意义;应根据药敏试验结果合理选择抗菌药物治疗;对于真菌性腹膜炎应尽早拔管行血液透析治疗。

Abstract:
[Abstract] Objective To evaluate the distribution and drug resistance of common pathogenic bacteria of infectious peritonitis related to peritoneal dialysis.Methods Sixty inpatients with peritoneal dialysis related infectious peritonitis from in our hospital June 2013 to October 2014 were selected as the research subjects and their clinical data were retrospectively analyzed.Results The microbial cultures were positive in 29 cases(48.33%), including 12 cases with Gram positive cocci, 14 cases with Gram negative bacilli and 3 cases with fungi; In Gram negative bacilli, the antibacterial drug with the highest resistance rate was Ampicillin(78.57%) and the antibacterial drug with the lowest resistance rate was Imipenem(16.67%). In Gram positive cocci, the antibacterial drug with the highest resistance rate was Penicillin(76.92%), followed by Ciprofloxacin(71.43%).Conclusion Both of Gram positive and Gram negative bacteria are the common pathogenic bacteria leading to the peritonitis caused by peritoneal dialysis. Antimicrobial agents should be selected according to the sensitivity of the drugs, Extubation should be performed as soon as possible for the patients with fungal peritonitis.
