| ［摘要］ 目的 探讨全髋关节置换术治疗中老年人股骨颈骨折的临床疗效。方法 将该院2012-01～2015-06收治的68例中老年股骨颈骨折患者按随机数字表法分为置换组（n=35）和内固定组（n=33），内固定组采取切开复位内固定手术治疗，置换组采取全髋关节置换术治疗，比较两组治疗效果。结果 置换组手术时间、术中失血量长于或多于内固定组（P<0.01），但卧床时间明显短于内固定组（P<0.01），两组住院时间比较差异无统计学意义（P>0.05）；置换组Harris评分为（73.15±5.27）分，内固定组为（52.08±3.62）分，两组比较差异有统计学意义（P<0.05）；两组并发症发生率比较差异无统计学意义（P>0.05）。结论 全髋关节置换术治疗中老年股骨颈骨折关节功能恢复更快、更好，可明显改善患者患肢功能，提高生活质量。 |

| ［Abstract］ Objective To investigate the clinical effect of total hip replacement in the treatment of femoral neck fracture in middle-aged and elderly patients.Methods Sixty-eight middle-aged and elderly patients with femoral neck fracture in our hospital from January 2012 to June 2015 were randomly divided into the replacement group(n=35) and the internal fixation group(n=33). Patients in the replacement group received total hip replacement, and the internal fixation group received internal fixation.The clinical effects were compared between the two groups.Results The average operation time was significantly longer and the intraoperative blood loss was more in the replacement group than those in the internal fixation group（P<0.05）. The bed rest time was significantly shorter in the replacement group than that in the internal fixation group（P<0.05）. There was no significant difference in the time of hospital stay between the two groups（P>0.05）. The Harris hip scores the replacement group（73.15±5.27） were more than those the internal fixation group（52.08±3.62）, P<0.05.Conclusion Total hip replacement is better than internal fixation for the treatment of femoral neck fracture in the middle-aged and elderly patients,with a quickly improved limb function and better quality of live. |
