高摄99mTc功能桥本甲炎甲减期患者甲状腺动态显像特点及临床意义

摘要:
[摘要] 目的 了解高摄99mTc功能桥本甲状腺炎(hashimoto thyroiditis,HT)甲减期患者甲状腺动态显像特点及其临床意义。方法 筛选60例HT甲减期患者,其中观察组30例摄99mTc功能增高患者,另30例摄99mTc功能减低或正常患者为对照组。所有患者行血清FT3 、FT4 、sTSH测定,99mTc甲状腺动态显像,观察甲状腺早期血流灌注峰值时间(early thyroid blood flow,EBF)、血流灌注指数(perfusion index,PI)、早期吸收指数1(uptake index 1,UI1)、早期吸收指数2(uptake index 2,UI2)及静态甲状腺摄99mTc指数的结果,分析EBF、PI、UI1、UI2与甲状腺摄99mTc指数及血清甲状腺激素之间的关系。结果 观察组EBF、PI、UI1、UI2值均明显低于对照组(P<0.05),EBF、PI、UI1与甲状腺摄99mTc指数呈负相关(r=-0.623,-0.3402,-0.5719,P<0.05),与sTSH呈正相关(r=0.441,0.503,0.473,P<0.05)。结论 高摄99mTc功能HT甲减期患者甲状腺动态显像特点为甲状腺血流灌注较摄99mTc减低或正常患者更为丰富,早期摄99mTc能力更强。从而推断甲状腺高血流灌注为造成HT甲减患者摄99mTc增高的原因之一。

Abstract:
[Abstract] Objective To study the characteristic and clinical significance of thyroid nuclear dynamic imaging in the patients with Hashimotos thyroiditis(HT) hypothyroidism with high thyroidal technetium-99m pertechnetate(99mTc) uptake.Methods Serum free T3, free T4, sTSH and EBF, PI, UI1, UI2, thyroid stactic uptake of 99mTc were dectected in 60 patients with HT hypothyroidism in whom 30 cases with high 99mTc uptake were taken as the study group, and the other 30 cases with normal or low 99mTc uptake as the control group.Results EBF, PI, UI1 and UI2 were significantly lower in the study group than those in the control group(P<0.05). There was a negative correlation between EBF, PI, UI1 and thyroidal 99mTc uptake(r=-0.623, -0.3402, -0.5719, P<0.05). There was a positive correlation between EBF, PI, UI1 and sTSH(r=0.441, 0.503, 0.473, P<0.05).Conclusion More blood to the thyroid gland and more uptake 99mTc in early duration are the characteristics of thyroid nuclear dynamic imaging in the patients with HT hypothyroidism with high 99mTc uptake. The results indicate that an increased blood flow to the thyroid gland is the mechanism of high thyroidal 99mTc uptake in the patients with HT hypothyroidism.
