脊髓麻醉与全麻对腰椎间盘突出手术患者术后疼痛及并发症的影响

摘要:
[摘要] 目的 探究脊髓麻醉与全麻对腰椎间盘突出手术患者术后疼痛及并发症的影响。方法 选择2015-07~2016-10在该院行腰椎间盘突出手术的患者80例,根据随机数字表法将所有患者分为观察组和对照组各40例。观察组实施脊髓麻醉,对照组实施全麻。比较两组术后1 h、24 h疼痛程度、麻醉满意度、手术时间、围术期和术后并发症发生情况。结果 观察组术后1 h、24 h视觉模拟评分法(VAS)评分均低于对照组,手术时间较对照组明显缩短,差异有统计学意义(P<0.01);观察组在围术期和术后恶心、呕吐及寒战的发生率明显低于对照组,差异有统计学意义(P<0.05);两组围术期和术后心律失常发生率比较,差异无统计学意义(P>0.05);观察组VRS评分优于对照组,差异有统计学意义(P<0.05)。结论 与全麻相比,对腰椎间盘突出手术患者实施脊髓麻醉,能有效减轻患者术后疼痛,缩短手术时间,降低并发症发生率,有助于加快患者预后恢复进程。

Abstract:
[Abstract] Objective To investigate the effects of spinal anesthesia and general anesthesia on postoperative pain and complications in the patients with lumbar disc herniation.Methods From July 2015 to October 2016, 80 patients with lumbar disc herniation in our hospital were divided into two groups according to the random number table, with 40 cases in each group. The observation group was treated with spinal anesthesia, while the control group was given general anesthesia. The pain scores 1 h and 24 h after the operation, the anesthesia satisfaction, the operation time, and the perioperative and postoperative complications were compared between the two groups.Results The postoperative VAS scores of 1 h and 24 h in the observation group were significantly lower than those in the control group, and the operation time of the observation group was significantly lower than that of the control group(P<0.01). The incidences of perioperative and postoperative nausea and vomiting and the incidence of shivering in the observation group were significantly lower than those in the control group(P<0.05). There were no significant differences in the incidences of perioperative and postoperative arrhythmia between the two groups(P>0.05). The VRS scores of the observation group were lower than those of the control group(P<0.05).Conclusion Compared with general anesthesia, spinal anesthesia can effectively reduce the postoperative pain, shorten the operation time and reduce the incidence of complications for the surgical patients with lumbar disc protrusion.
