加长型PFNA及股骨近端锁定钢板治疗股骨粗隆下骨折的临床效果比较

摘要:
[摘要] 目的 比较加长型股骨近端防旋转髓内钉(proximal femoral nail anti rotation,PFNA)及股骨近端锁定钢板治疗股骨粗隆下骨折的临床疗效。方法 回顾性分析2011-09~2015-08在该院行手术治疗的65例股骨粗隆下骨折患者的临床资料,其中包括采用加长型PFNA治疗的患者31例(加长型PFNA组)和采用股骨近端锁定钢板治疗的患者34例(锁定钢板组)。记录每例患者的手术时间、术中出血量、术后完全负重时间、骨折愈合时间及术后颈干角丢失的情况,利用Harris评分评估术后6个月时的功能。结果 两组患者随访时间为11~25个月,平均16个月。两组Harris评分差异均无统计学意义(P>0.05)。加长型PFNA组手术时间、完全负重时间、骨折愈合时间均短于锁定钢板组,出血量少于锁定钢板组,颈干角丢失程度低于锁定钢板组。结论 加长型PFNA治疗股骨粗隆下骨折具有手术创伤小、出血量少、允许患者早期下地、固定牢靠的优点,有较好的疗效。

Abstract:
[Abstract] Objective To compare the clinical effects of long proximal femoral nail anti-rotation(PFNA) and proximal femur locking plate on the treatment of subtrochanteric fracture.Methods The clinical data of 65 patients with subtrochanteric fracture in our hospital from September 2011 to August 2015 were retrospectively analyzed, among whom 31 cases received long PFNA and 34 cases were treated with proximal femur locking plate. The operation time, intraoperative blood loss, full weight bearing time, fracture healing time and neck-shaft angle discrepancy were analyzed. The femoral head function was assessed by Harris scores system 6 months after the operation.Results The average follow-up period was 16 months(ranges from 11 to 25). There were no significant differences between the long PFNA group and the proximal femur locking plate group in the general condition and Harris scores(P>0.05). The long PFNA group had shorter operation time, full weight bearing time and fracture healing time, less intraoperative blood loss and less neck-shaft angle discrepancy than the proximal femur locking plate group.Conclusion Long PFNA has the advantages of less invasion and intraoperative blood loss, earlier walk,firmer fixation and better clinical effect than the proximal femur locking plate in the treatment of subtrochanteric fracture.
