脑梗死患者血清Cat S及Cys C水平与颈动脉狭窄程度的相关性

摘要:
[摘要] 目的 探讨脑梗死患者血清组织蛋白酶S(Cat S)及胱抑素C(Cys C)水平与颈动脉狭窄程度的相关性。方法 将336例动脉粥样硬化性脑梗死患者,依据颈部血管超声测定结果,分为轻度狭窄、中度狭窄和重度狭窄3组。同期114例健康体检者为对照组。所有入组者第2天和第14天均空腹采血检测血清Cat S及Cys C。结果 轻度狭窄组、中度狭窄组及重度狭窄组与正常组比较,血清Cat S、Cys C水平及两者的比值Cat S/Cys C均有显著差异(P<0.05);血管狭窄程度越重,Cat S及Cat S/Cys C增高越明显、Cys C降低越明显,差异均有统计学意义(P<0.01),经Spearman相关分析,血管狭窄程度与血清Cat S水平呈正相关(r=0.475,P<0.05);第14天时,各组血清Cat S及Cat S/Cys C均较入院时降低,而Cys C较入院时增高,差异均有统计学意义(P<0.05)。结论 脑梗死患者血清Cat S与颈动脉狭窄程度呈正相关,Cys C与颈动脉狭窄程度呈负相关,Cat S/Cys C比值变化更明显,是颈动脉狭窄程度及脑梗死发生的预测指标。

The relationship between cathepsin S/cystatin C and the stenosis degree of carotid artery in cerebral infarction patients

Abstract:
[Abstract] Objective To investigate the relationship between cathepsin S(Cat S)/cystatin C(Cys C) and the stenosis degree of carotid artery in cerebral infarction patients. Methods 336 patients with atherosclerosis cerebral infarction were recruited and were divided into mild stenosis, moderate stenosis and severe stenosis groups according to different stenosis intensities. During the same period, 114 healthy examinees were recruited as the health controls. The bloods of all the patients and the control group were collected and the Elisa and latex-enhanced immunoturbidimetric assay methods were used to detect the serum levels of Cat S and Cys C respectively at day 1 and 14 after stroke occurance. Results The levels of Cat S, Cys C and ratio of Cat S/Cys C had significant differences(P<0.05) between the mild stenosis group, the moderate stenosis group, the severe stenosis group and the control group. With the increase of vascular stenosis, the levels of Cat S and ratios of Cat S/Cys C were positively and Cys C was negatively correlated with the vascular stenosis severity. Spearman correlation analysis showed that the level of Cat S was positively correlated with the unstable plaque(r=0.475, P<0.05). 14 days after stroke, the levels of Cat S and ratios of Cat S/Cys C in all the patients were relatively reduced and the levels of Cat C were higher compared with those 1 day after stroke(P<0.05). Conclusion In the patients with cerebral infarction, Cat S is positively correlated with the severity of vascular stenosis, and Cys C is negatively correlated with the severity of vascular stenosis. The changes of Cat S/Cys C are more significant in the patients which are the predictors for diagnosis of cerebral infarction and the severity of vascular stenosis.
