[摘要] 目的 对比MRI和二维超声、三维Smart MSP对胼胝体发育异常的诊断价值。方法 选择产前超声诊断怀疑胎儿胼胝体异常的孕妇20例,分别进行二维超声、三维Smart MSP和MRI检查,与MRI结果对比,分析二维超声和三维Smart MSP诊断胼胝体异常的检出率及准确性。结果 三维Smart MSP超声对胼胝体异常(完全性胼胝体缺如+部分性胼胝体缺如)的检出率显著高于二维超声(P<0.05)。三维Smart MSP检出胼胝体异常的灵敏度、特异度均高于二维超声。三维Smart MSP在诊断胼胝体异常病例中与MRI有很好的一致性(Kappa=0.844)。结论 使用三维Smart MSP技术能达到接近MRI诊断胼胝体发育异常的水平,具有较高的临床应用价值。 |

[Abstract] Objective To compare the diagnostic value of MRI, two dimensional ultrasound(2D-ultrasound) and three dimensional ultrasound(3D-ultrasoud) Smart-MSP(median sagittal plane) in dysplasia of the corpus callosum. Methods Twenty fetuses suspected to have dysplasia of the corpus callosum by antenatal diagnosis with ultrasound were selected and performed prenatal examination using 2D-ultrasound, 3D-ultrasoud Smart-MSP and magnetic resonance imaging(MRI) respectively. The examination results were compared among the three methods, and the detection rate and the diagnostic accuracy rate were compared between the patients with MRI examination and the patients with 2D-ultrasound or 3D-ultrasoud Smart-MSP examination. Results Compared with 2D-ultrasound, 3D-ultrasound Smart MSP had a significantly higher detection rate in diagnosis of dysplasia of the corpus callosum,including complete agenesis of the corpus callosum and partial agenesis of the corpus callosum(P<0.05). The sensitivity, specificity of 3D-ultrasound Smart MSP were higher than those of 2D-ultrasound. 3D-ultrasound Smart MSP was in good agreement with MRI in diagnosing dysplasia of corpus callosum(Kappa=0.844 ). Conclusion 3D-ultrasound Smart MSP is close to MRI in diagnosing dysplasia of the corpus callosum and has high clinical application value. |
